NURS 2021H Clinical Course Mid-Term Evaluation

Course Objective
Recognize and begin to navigate the
complexities of family nursing.

Progress
Evidence/Indicators
The following examples demonstrate
how I have recognized and began to
navigate the complexities of family
nursing:
As a nursing student, I have come to
learn the significance of developing a
therapeutic relationship with all
clients and their families and that
relational inquiry is essential in family
focused nursing.
I understand that providing care for
clients and their families can be a
complex and challenging task. Over
the course of the past few weeks, I
have placed a lot of emphasis on
learning how I can target specific
goals, assets and health requirements
for each client and their support
systems.
I have demonstrated competence in
my practice by setting my personal
biases, assumptions and beliefs to the
side. This has allowed me to have an
open mind and successfully treat all
clients equally, no matter their family
history.
I appreciate all that I have learned so
far, however one of the most
important things I have come to
realize is that family and mental
health nursing is a never ending

learning process. I look forward to

future placement days where I can
develop new skills that will assist me
to further navigate my way though
family nursing.

Integrate knowledge from previous courses The following examples demonstrate
how I have integrated knowledge from
to support diverse populations.
previous courses to support diverse
populations:
NURS 1002 has provided me with a
strong understanding of the many
foundational concepts, such as,
nurse-client relationships, relational
inquiry and person-centered relational
practice. These concepts have
allowed me to build relationships with
a variety of different types of clients
over the past few weeks.
PHIL 2390 has provided me with
crucial information in relation to
ethics. It is important to understand
how to act morally when working with
diverse and vulnerable populations.
There are many different belief
systems that I have encountered and
will continue to encounter.
A specific lecture from NURS 1000
focused on psychosocial health and
its components: emotional health
(feeling), spiritual health (being),
mental health (thinking) and social
health (relating). I believe that all of
these components play a major role
in mental health and mental health
nursing, so having previously learned
about them has provided me with a
strong foundation to work from. I
have made a conscious effort to

Critically appraise relational inquiry

processes and begin to develop
meaningful relationships with health care
providers and family members.

incorporate all of these aspects into

my practice thus far.
All of these courses have provided me
with a lot of useful information that has
allowed me to be successful in my
practice.
The following examples demonstrate
how I have critically appraised relational
inquiry processes and began to develop
meaningful relationships with health
care providers and family members:
On Friday June 3, 2016 I developed a
meaningful relationship with one of
the male clients on the unit. I
developed a professional and trusting
relationship with him, allowing him to
feel comfortable with my presence.
This client and I, along with two of my
colleagues, played a game of
cribbage. This situation allowed me to
build a meaningful relationship with
this client, as well as strengthen the
relationships I have with my
colleagues.
By critically appraising the relational
inquiry process I have learned that it
allows nurses to recognize similarities
and differences in people, yet still
develop relationships despite the
differences. As a student nurse, I am
making a conscious effort to
recognize similarities and differences
in the people that I work with. A
relational approach will encourage my
personal growth, enhance my current
knowledge and allow me to better
respond to colleagues, clients and
families.

Collaboratively formulate a plan of care

based on knowledge of family nursing,
related theories and scholarly literature.

The following examples demonstrate

x
how I have collaboratively formulated a
plan of care based on knowledge of
family nursing, related theories and
scholarly literature:
By completing rounds I am actively
participating in formulated care plans;
these rounds ensure the safety of the
clients. For example, clients on close
observation are at greater risk of selfharm; therefore by charting their
location every 15 minutes I am
ensuring that they are as safe as
possible.
On June 11, 2016 a colleague and I
took blood pressure measurements of
a male client, one in a seated position
and one in a standing position. This
patient is currently on a medication
that can alter his blood pressure. By
taking this measurement I was
helping to ensure that this client is
healthy, safe and that his medication
is at the right dose and working
effectively.
By helping with the breakfast trays I
am assisting in the clients nutritional
plans of care. Ensuring that clients
are starting their days with a
nutritious meal can improve and
promote overall health and aid in
their recovery processes.

Develop, implement and evaluate the

effectiveness of health-promoting,
evidence-based practice, reflecting
principles of family nursing as relational
practice.

The following examples demonstrate

how I have developed, implemented and
evaluated the effectiveness of health
promoting, evidence-based practice,
reflecting principles of family nursing as
relational practice:
On our orientation day we were
introduced to the nurse that is in
charge of the students at the inhospital school. Learning about her
role and watching her interact with
the clients allowed me to understand
how significant her role in health
promotion is. These children are
dealing with mental illnesses that
have affected their ability to remain
in school; she plays a major role in
assisting these children in continuing
their education.
There is evidence to show that
supportive interpersonal therapeutic
relationships are important in mental
health as it assists in the growth of
the client. I have had the opportunity
to witness nurses on the unit interact
with clients in a therapeutic manner. I
personally have also interacted with
clients in a therapeutic manner. By
interacting in this way, we are
developing trusting relationships with
the clients in hopes that we can help
them on their road to recovery.
I have had the opportunity to
participate in activities that promote
health. Gardening and physical
activities help encourage clients to
realize that health is more then just
taking medications to feel better.

Being physically active and engaging

in social activities contributes to
holistic health.
This is a goal that I would like to focus
on more over the course of the rest of
the summer.
Demonstrate increasing competence and
confidence in the application of
psychomotor skills in practice settings.

The following examples demonstrate

how I have demonstrated increasing
competence and confidence in the
application of psychomotor skills in
practice settings:
I displayed competence and
confidence in my psychomotor skills
on June 11, 2016. I accompanied two
clients out to the courtyard for
gardening. I interacted with the
clients, while also planting flowers.
This allowed me to bond with the
clients, improve my relationships with
my colleagues and promote the
health of these clients by exposing
them to fresh air and relaxing
activities.
I applied psychomotor skills in the
practical setting when I took blood
pressure measurements on a client.
This task required me to use my
motor skills; it required that I
precisely place the cuff on the clients
arm for an accurate reading.
I demonstrated increasing
competence and confidence on June
3, 2016 when I participated in
physical activities while on the
adolescent unit. These activities
included bocce ball, volleyball and
badminton. I tried to take initiative

and encourage the adolescent clients

to participate in the activities.
x
Demonstrate accountability and
The following examples demonstrate
professionalism that is consistent with a
how I have demonstrated accountability
nurse entering a self-regulating profession. and professionalism that is consistent
with a nurse entering a self-regulating
profession:
I display accountability and
professionalism when I sign out
clients charts. I am acting in a
professional manner by reading
through the charts in a private room,
maintaining confidentiality and
withholding any form of judgment
about the clients. I am accountable in
that I am responsible for the location
of that chart for the entire time I have
it signed out.
I display accountability when I
complete my rounds and sign
off/initial the form. By placing my
name on this sheet, I recognize that I
am responsible for the information
that I have recorded.
I displayed accountability and
professionalism on June 11, 2016.
After reading through a clients chart I
came to the realization that I am
familiar with the client and his family.
I approached my instructor about this;
I wanted to be accountable in case
there was any conflict of interest.
On June 11, 2016 I displayed
professionalism by taking on the role
of team leader. Throughout the
course of this day I was
accountable/responsible for my

colleagues and needed to be

available in case they needed
assistance.
Select appropriate community support
services for families needing referral to
enhance coping with diverse transitional
experiences.

The following examples demonstrate

how I have selected appropriate
community support services for families
needing referral to enhance coping with
diverse transitional experiences:
On June 11, 2016 we had a guest
speaker (Scott) talk to us about his
lived experience; this presentation
had a large impact on me. Scott
introduced us to a resource known as
211. It is a reference that directs
individuals to various community and
social services across Ontario. I can
now offer this information to others,
or use it for myself if I ever feel the
need.
On mental health day at Trent
University we learned about various
resources within our community, such
as Fourcast and One Roof Community
Diner. Now that I have the knowledge
about resources such as these I can
educate others about them. I look
forward to July 1, 2016 when my
colleagues and I will attend the One
Roof Community Diner.

After the completion of this chart, I think that I have been successful in my progress thus far. I have provided
specific examples for each objective, however I look forward to the remainder of the semester as it will allow me
increase my knowledge and improve my skills.
