AFFIDAVIT

LOST

I, JOSEPH P. MORDENO, of legal age, married, a resident of 620-S M. J

Cuenco Avenue, Cebu City. KEN LLOYD N. MORDENO is my son a student
in University of Cebu. I sworn in accordance with the law depose and say:

That my son a holder of University of Cebu School I.D ;

2. That on or about July 20, 2016, his School I.D was lose in the
following manner:
3. When he was about to go home riding a PUJ Vehicle, he brought all
pertaining school
documents as needed in his daily school activities,
suddenly when he arrived
home he notice that one of his folder was no
longer with him, which his school I.D was also on that folder and its
whereabouts cannot be ascertained despite due and diligent
search
made and undertaken.

That I now desire and hereby request the University of Cebu to
issue a new I.D for
my son
for SY 2016 -2017, in lieu of the one lost;
that I am willing to surrender the lost
I.D in the event that same could be
found and located; that I bind myself to hold
UNIVERSITY OF CEBU free
and harmless from any loss or damage that may arise or it may
incur
in
connection with the issuance of the I.D; that each and all of the facts herein
stated are true; and that I am willing to testify before any court of
justice to substantiate the truth of each and all of the above said facts.
