PETITION

COMES NOW the PETITIONER, ATTY. DEARLIT JANE

C. GOLO, unto this Honorable Court, most respectfully files
this application for her Commission as a Notary Public for
the City of Davao. In support of this petition, she alleges
that:
1. Petitioner possesses the eligibility for commissioning as
Notary Public as shown by her following personal
circumstances:

2. Petitioner has satisfactorily completed her Bachelor of
Laws at the Ateneo de Davao University College of Law
in the year 2013;

3. Petitioner passed the Bar Examinations of October 2013

and admitted to the practice of law in April 15, 2013.
She is listed under the number 78232 in the Roll of
Attorneys;
4. Petitioner is a member of the Integrated Bar of the
Philippines, Davao City Chapter, with Roll Number
5. Petitioner holds office at the following address:

6. Petitioner has not been convicted, much less, charged
with any offense involving moral turpitude;
7. Petitioner has not applied for nor was she issued a
Commission as a Notary Public in any other jurisdiction;
8. Petitioner is desirous of being commissioned as Notary
Public for and in Davao City for a period of two (2)
years, starting February 15, 2018 up to December 31,
2020;
9. Petitioner undertakes to submit or report to the Office
of the Clerk of Court copies of all deeds notarized by
her.
PRAYER
CONSIDERING ALL THE FOREGOING, it is
respectfully prayed of the Honorable Executive Judge of the
Regional Trial Court, Branch 11, Davao City to grant the
notarial commission of herein petitioner Atty. Dearlit Jane C.
Golo;

single and a resident of 256, Solar Street, GSIS Heights
Subdivision, Matina, Davao City, after being duly sworn to in
accordance with law hereby state that:
1. I am the petitioner of the above instant case;
2. That I have caused the preparation of the aboveentitled petition;
3. That I have read the same and that it is true and
correct based on my personal knowledge or based on
authentic records.
4. That I hereby certify under oath in this petition as
follows:
a. I have not theretofore commenced any action or filed
any action or claim involving the same issue in any
court, tribunal or quasi-judicial agency and, to the
best of my knowledge, no such other pending action
or claim is pending therein;
b. If there is such other pending action or claim, a
complete statement of the present status thereof;
and

c. If I should thereafter learn that the same or similar

action or claim has been filed or is pending, I shall
report that fact within five (5) days therefrom to the
court wherein the aforesaid complaint or initiatory
pleading has been filed.
