AFFIDAVIT
I, Dr. Henry C. Lee, of legal age, single, residing at 82 Limewood Ave.
Branford, CT 06405 122, after being sworn to in accordance with the law,
depose and say:
1. That on February 3, 2014, I was a consultant conducting training to the
pertinent officers (since January 10, 2014) at the PNP Crime
Laboratory;
2. That at about 8:30 a.m. of the same day, two bodies, later identified as
Fe Rafael, and Danilo Rafael, Jr., were brought to our laboratory for full
autopsy.
3. That I am the forensic expert who personally attended to the autopsy
of the victims on the same day;
4. That after conducting the autopsy, the cause of death of the victim
Danilo Rafael Jr. was found to be a gunshot wound on the frontal skull
bone;
5. That after conducting the autopsy, the cause of death of the victim Fe
Rafael was also a gunshot wound on the frontal skull bone;
6. That the victim Danilo Rafael Jr. also had a collapsed left lung which is
caused by pneumo hemothorax secondary to gunshot wounds on the
left lung; and
7. That the victim Danilo Rafael Jr also had several other gun shot wounds
in the clavicle area and the abdomen which caused damage to his liver.
8. That my findings and diagnosis are placed in my summary autopsy
reports of the two bodies, with their corresponding body sketches
which documents are hereto attached as Annexes _, _, _, and _.
