disminucinde la vasoconstriccin en la
arteriola posglomerular, con la consiguiente reduccin de la presin intraglomerular, y el segundo seria un efecto directo al variar la selectividad de la membrana y su permeabilidad [27] y evitar el
paso de albmina a travs de la misma. Al
parecer, este efecto es independiente del
efecto hipotensor [28J. Se ha demostrado
que la respuesta a los ARA I1 en el diabtico es muy rpida. Incluso en el periodo
de siete das se ha observado una importante disminucin de la microalbuminuria tras el inicio del tratamiento con losartn [29]. Tambin algunos trabajos sugieren que lasuma de ambos frmacos-lECA
y ARA Il-puede tenerun efecto sinrgico
al precisarse menos dosis de ambos para
frenar la progresin de la nefropata [30J.
Parece tan demostrado el efecto beneficioso de estos agentes sobre la alteracin renal que, en casos extremos en
normalmente tienecarganegativa,como
la albmina. Este cambio en la polaridad
de la membrana permite el paso de la albmina a su travs, y este inicio de la al-
o ARA 11 [31].
No obstante, el tratamiento con lECA
o ARA 11 no protege totalmente el rin
en el paciente diabtico o hipertenso, y
