Registration certificate from the Securities and Exchange Commission (SEC),


Department of Trade and Industry (DTI) for sole proprietorship, or Cooperative
Development Authority (CDA) for cooperatives.
Mayor's permit issued by the City or Municipality where the principal place of
business of the prospective bidder is located;
Pursuant to GPPB Res. No. 29-2012, ITB 12.(a)(iii) is hereby revised and the following
requirements shall be submitted. Otherwise, the bid(s) shall be rejected or disqualified:

Audited Financial Statements, showing, among others, the prospective total current assets
and liabilities stamped "received" by the BIR or its duly accredited and authorized institution
for the preceeding calendar year which should not be earlier than two (2) years from the da
of bid submission;
The prospective bidder's computation foi its Net Financial Contracting Capacity (NFCC) or a
commitment from a Universal or Commercial Bank to extend a credit line in favor of the
prospective bidder if awarded the contract to be bid (CLC).

Class "B" Documents:


If applicable, valid Joint Venture Agreement (JVA) or, in lieu thereof, duly notarized
statements from all potential joint venture partners stating that they will enter into and
abide by the provisions of the JVA in the instance that the bid is succesful shall be included

in the bid.
Technical Documents
The bidder shall submit the Bid Security in the form of Bid Securing Decalaration (BSD) and
any of the following forms:

1. Two percent (2%) Cashier's/Manager's Check issued by a Universal or Comme

2. Two percent (2%) Bank draft/guarantee or irrevocable letter of credit issued by
Universal or Commercial Bank: Provided, however, that it shall be confirmed o
authenticated by a Universal or Commercial Bank, if issued by a foreign bank

3. Five Percent (5%) Surety bond callable upon demand issued by GSIS or any su
insurance company duly certified by the Insurance Commission

Conformity with the technical specifications, as enumerated and specified in Sections VI and
VII of the Bidding Documents (Sample of the furniture to be delivered)

Sworn statement by the bidder or its duly authorized representative in the form
prescribed by the GPPB as to the following:
a. It is not "blacklisted" barred from bidding by the GOP or any of its agencies,
offices, corporations, or LGUs, including foreign government/foreign or internation
financing institution whose blacklisting rules have been recognized by the GPPB;
b. Each of the documents submitted in satisfaction of the bidding requirements i
authentic copy of the original, compelete, and all statements and information
provided therein are true and correct;
c. It is authorizing the Head of Prcuring Entity or his duly authorized represenata
to verify all the documents submitted;
d. The signatory is duly authorized representative of the prospective bidder, and
granted full power and authority to do, excute and perform any and all acts nece
and/or to represent the prospective bidder in the bidding, with the duly notarized
Secrearary's Certificate attesting to such fact, if the prospective bidder is a corpo
partnership or joint venture;
e. It complies with the disclosure provision under Section 47 of the Act in relation
other provsions of RA 3019;
f. It complies with the responsibilities of a prospective bidder provided in the PBD
and
g. It complies with the existing labor laws and standards.
h. That did not give or pay directly or indirectly, any commission, amount, fee o
any form of consideration, pecuniary or otherwise, to any person or official,
personnel or representative of the government in relation to any procurement
project or activity.
