a PVC hose pipe. Although mostly fresh water is injected
into tank, sometimes phenol water wash is also
discharged through the same set up. Its a nonconductive hose as well the strength & integrity of the
same can not be relied upon. It should have a set-up
similar to that being used in tank-102.

2 Castor oil tankers offloading was found with a hose

Operation was stopped and asked them to tighten all

OS internal at main junction has its valve half inside the
the ground/flooring, in case of any leakage/failure
around the gasket during transfer or shipment it will be
very hard to arrest

While pigging was in progress from OJ-2 for BAM in

8"GREEN x 8"GREEN line there was a spark observed at
main junction. The spark supposedly came from shortcircuiting of wire of one of the lights. It was observed by
GET SAHIL SINGH & junction workers. Immediately the
incident was reported to shift leader & Wire man. After
which the supply of lighting to main junction was turned
off from main substation. The incident occurred around
0115 Hrs.

terminal at 9.50 AM, as soon as knew near Weigh bridge
that mobile is with me, immediately went to office and
kept the mobile in office. This will not happen again by

Earthing copper jump over found idle on ground in

Tanker no-GJ12AT 7682 Over flow 5 to 6 litre from
compartment no-1 during loading. Transporter J.B.Roadlines, Tank no- 107 Product- N.Butanol
During start up of external transfer of cyclohexanone
from tank no 119 to FOCT tank no 114, gasket got
damaged in 8" green dock line pig receiver tapping at
main junction where discharge hose of pump was
connected.Pumping was stopped immediately and
gasket was changed after emptying hose by air
blowing.Spillage was approx. 5 liters
During start up of Internal transfer of solvent naptha l
(from tank-112 to tank-161) leakage observed from 6''
hose(hose no-245) connected in delivery of centrifugal
pump. Pumping was stopped immediately .Instantly
6"hose replaced (hose no-336).
GJ 12 W 7784 & GJ12W 7065 tankers are parked under
gantry for unloading of cargo but unloading not going
on. . Observed tanker drivers sit on pavement without
wearing of safety glasses. Intervene the act, by made
understand them for wearing of glasses and to use the
driver sitting shed when tanker unloading not goin.
