Subjective Data:
The mother says
the doctor came
in and said
Dillon's
"eso..esopha..jus
wasn't formed
right...what does
this mean? Is my
baby going to
die?"
Objective Data:
Cyanotic
Coughing
Choking
Excessive
amounts of
saliva and
mucus
V/S:
T- 37.1
P- 120 bpm
B/P- 105/60
R/R- 10 bpm

Diagnosis
Impaired
gaseous
exchange related
to abnormal
opening between
esophagus and
trachea as
evidenced by
cyanosis.

Planning
At the end of
two hours, the
babys will have
an oxygen
saturation of
99%.

Implementation
Oxygen Therapy
via nasal
cannula at 2L/hr.
r- Provide
adequate oxygen
and eliminate
signs of
cyanosis.
Suction the
babys mouth
and nose.
r- To relieve and
prevent airway
obstruction.
Place baby in a
semi fowler
position.
r- Allow full
lung expansion
to assist baby
with breathing.
NPO until cause
of distress is
known
r- Prevent
airway
obstruction and
hypoxia

Evaluation
At the end of
two hours, the
baby had an
oxygen
saturation of
99%. Baby skin
was not blue,
indicating no
sign of cyanosis.
