If you are interested in being treated with HSCT - apply! Even if you are not sure if you are a candidate, it does
not hurt to take the first step.
To Apply for the study:
Typically, the initial contact to see if you may qualify for the study is done via email.
The initial contact email should be directed to Kathleen Quigley at k-quigley@northwestern.edu.
In your initial email, include the following information:
1. You are interested in applying for HSCT
2. What disease(s) you have
3. Date of diagnosis
4. Current symptoms
5. Medications you've tried since diagnosis and for how long (Approximate dates are helpful. If you don't
remember specific dates, at least try to put the year. i.e. Started Copaxone 2005, stopped 2009 = 4 years).
6. Did those medications work? What symptoms did you have? Why did you stop them?
7. List your current medications and when you started them.
8. For MS when were your last two exacerbations (relapses)?
9. If you have MS & know your EDSS score give them that.
10. Indicate other health issues that you have.
11. You won't need to send records at this point, but it might be helpful to start gathering your records so you'll
have them ready to send when they request them.
12. Check your email (and junk/spam folder) daily for their reply. Once they receive your email, they will
respond letting you know whether they think you may or may not qualify. If they think you may qualify, they will
send you the forms to complete and ask for your medical records. If you do not hear from them within two
weeks, send your initial email again or call the number below to confirm that they received your initial email.

If you are a candidate for an evaluation


o Make a timeline of your diagnosis: MRIs, exacerbations, medications, any surgeries you have had.
o Make a list of your current symptoms.
o Detail your questions for Dr. Burt with spaces below each question to write notes. Do NOT ask if
you can be treated on a compassionate basis. Dr. Burt will offer that if he thinks that you may
benefit from the treatment, even if you do not meet the criteria for the study.
o Bring these documents to your evaluation they will help out both you and Dr. Burt. The time
with Dr. Burt during evaluation will go fast, so make sure you speak up and ask your questions!
