Review of Systems

General
: Has lost weight before admission and experienced weakness,
fatigue and fever.
Skin
: No observable rashes, lumps or sores or other changes
HEENT
Head : No history of head injury but experiences slight headache,
dizziness or
lightheadedness every after hours of sewing activity
Eyes : Have problems in vision sometimes blurry or double vision
every after hours of
sewing activity. Uses no glasses or contact lenses. No pain,
redness, or excessive tearing. No glaucoma or cataracts.
Ears : Slight problems in hearing but no ringing (tinnitus) vertigo and
infections.
Nose : Occasional mild cold. No hay fever and sinus trouble
Throat
: No significant bleeding. No dentures. Last dental
examination unknown. No
problems with dry mouth.
Neck : No lumps, goiter, pain. No swollen glands.
Breast
: No lumps, pain, discomfort and discharge. Practice breast
self-examination
infrequently.
Respiratory : Moderate cough without wheezing. No shortness of breath.
Chest x-ray, August
2016, at MMMH, Batac City (Normal findings)
Cardiovascular
dyspnea,

: No known heart disease or high blood pressure. No

orthopnea, chest pain, palpitations. Normal

Electrocardiogram (ECG)
taken on August 2016.
Gastrointestinal: Has abdominal pain located at RLQ. Loss of appetite PTA
and also after
medications. Slight belching or passing gas. No nausea,
vomiting, indigestion. No changes in bowel movement. No
diarrhea or bleeding, jaundice, gallbladder or liver
problems.
Urinary
: Frequency of urination changed after the operation. No dysuria,
hematuria, or
recent flank pain
Genital
: Menarche at 13 y/o. Menapause at 46 y/o No vaginal or pelvic
infections. No
dyspareunia. Had 7 pregnancies (NSD)
Peripheral
Vascular : No varicose veins. Experiences leg pain or cramps. No swelling
in the calves,
legs or feet. No changes in color of fingertips or toes during
cold weathers.
Musculoskeletal: No muscle or joint pain. No stiffness, arthritis, or gout. No
back pain. No pain
in neck or lower back.
Psychiatric: No history of depression or treatment for psychiatric disorders.
Neurologic: Has fainting experiences during the anxious moments of family
problems, No
seizures. No motor or sensory loss. Memory is remarkably good.
Hematologic: No easy bleeding. No history of anemia.
Endocrine: No known thyroid disorders or heat or cold intolerance. No

symptoms
or history of diabetes. Experiences mild sweating.
