If any opening made in the floor & same is barricaded
Persons are aware of theree point contact & use of hand rail & implementing the same
The safety belt in use are full body harness & with double lanyard
Is the safety belt properly hooked w.r.t to strength & location of anchor point
Is the width to height ration(minimum 1:4) maintained while ercting scaffold
The scaffold is erected on base plate/ Mud sill
Is the scaffolding in plumb & level
The scaffolding material is in good condition
