Safety Workshop 2010

Sustain Sound Behavioral Based Solution Culture with Tested & Proven Safety Principles

JW Marriott Hotel, Kuala Lumpur, Malaysia 29th 30th March 2010

More than 80% of on-the-job injuries are due to unsafe behaviors, rather than wrong devices
or unsafe conditions.
I am convinced that nothing we do is more important than hiring and developing people.
At the end of the day you bet on people, not on strategies.
~ Larry Bossidy, Chairman and CEO of Honeywell International, Inc

Workshop Overview
A fool-proof safety program will never be successful without having
employees involvement and changing their mindset. Behavioral
Based Safety (BBS) analyzes the interactions between peoples
behavior and the working environment to identify those factors that
support safe or unsafe behavior. By changing the environment to
support safe behavior and implementing proven behavioral safety
processes in your company, BBS aims to dramatically reduce the
number of lost-time and injuries.
The 2-day workshop is specially designed for safety professionals
who have an existing knowledge of BBS and it places a much
stronger focus on human factors and process safety issues. The
result shall be a much wider response than behavioral observation
process.
Led by our course facilitator, Mr. Charles Shoesmith, a highly
experienced psychologist who has wide experience of consulting
in BBS implementation in Europe, Asia and Africa, in sectors such
as Oil & Gas, energy, manufacturing etc, the workshop will include
a blend of theory and practice, emphasizing on applying behavioral
science. The delegates will leave the workshop with a changed
mindset, as well as a set of practical tools to apply immediately in
their daily work.

WHY YOU SHOULD ATTEND

This highly impactful workshop will take a holistic approach to
tackling the problem of unsafe behavior. Delegates will learn the
need to think beyond observation and coaching systems. The
approach will incorporate the need to focus on key issues such as
human factors, process safety and safety leadership. Delegates
will experience a highly interactive event case studies, activities
and the use of different media will all be featured. With the guidance
of our professional trainer, Mr. Charles Shoesmith, delegates will
have time to critically review where they are in relation to BBS
and where they might go next to realize zero injuries in their
organization.

DAY 1 | 29th March 2010

UNDERSTANDING THE HUMAN BEHAVIOR PROBLEM
SESSION 1: THE BBS CONTEXT
Difference between BBS and traditional safety approaches
Early approaches to BBS behavioral approaches involving
observation, coaching, recording
The need to understand why people do the things they do
Advanced BBS a more sophisticated approach to the
problem incorporating behavioral safety, human factors,
process safety etc
Establishing the business need convincing senior
management to make the investment
SESSION 2: BARRIERS TO EFFECTIVE IMPLEMENTATION
Not understanding the problem
Poor implementation design
Underestimating resources and effort required
A lack of trust and understanding
Not engaging the key stakeholders
Not enough emphasis on follow-through and reinforcement
SESSION 3: INCIDENT CAUSATION
The link between behavior and incident causation
The limitations of a traditional approach to BBS
Why dont we learn? Repeating patterns in major accident
events
Active and latent factors in incident causation
Our preference for simple explanations
The complexity of causation
Case studies / activities will include Texas City report and a
Coroners Court video
SESSION 4: HUMAN FALLIBILITY
What is human factors?
How reliable is the human being? The use of human
reliability analysis (HRA)
A model of human failure the interaction of the human
being with the environment
Individual differences can we select safe people?
Understanding the link between attitude and behavior
The difference between errors and violations
Cognitive limitations and how they can lead to error
Different error types and the implications for design etc
Different violation types
Activity will include determining different error / violation
types from a written scenario, assessing error / violation
probability for different tasks, and video of an incident in a
power plant
SESSION 5: HUMAN PERFORMANCE WHAT CAUSES
ERRORS / VIOLATIONS?
Learning to be safe - society vs company norms
Safety culture and safety leadership creating the context in
which people behave
Error provoking factors
Why do we choose to take risks? Reinforcement Theory
Factors that make violations more likely
High-risk situations
Activity: ABC Analysis using behavioral analysis to
understand risk taking behavior
SESSION 6: INCIDENT INVESTIGATION EXERCISE
This will involve working on a given case study, analyzing witness
statements etc to determine incident causation and offering
explanations involving descriptions of errors and violations, and
the linkages between how different people behaved

DAY 2 | 30th March 2010

IMPLEMENTING BBS
SESSION 7: REINFORCING THE DAY 1 LEARNING
What were the key messages / learning
What do these mean for designing and implementing BBS

CONTEXT OF SAFETY
Why is safety often viewed negatively? What are the
implications?
What practices demotivate employees?
Reinforcement Theory revisited a theory of motivation
Reward and recognition should this feature within BBS?
Measuring performance what is best practice?
SESSION 9: SAFETY BY DESIGN
How poor design causes accidents - what do accidents tell
us?
How poor design can demotivate and lead to risk taking
Designing the workplace to reduce the probability of human
error
Activity: designing the cockpit of a solar powered
helicopter
SESSION 10: SAFETY LEADERSHIP
Social Learning Theory how it applies to BBS
How leaders play an important part in shaping the culture
The zero harm vision and how to get others to buy in to it
What influences people what they are told or what they
directly experience?
The problem with mixed messages espoused theories vs
theories in action
Moments of truth what people take notice of
What do good safety leaders do? Activity
Safety is a dynamic non-event the need to keep safety
alive
The PsychaLogica Time For Safety process
Activities: will include group discussion on what good
safety leaders do, and individual work on completing Time
for Safety Matrix
SESSION 11: WORKFORCE INVOLVEMENT IN BBS
Why is safety often viewed negatively? What are the
implications?
Creating the right culture to promote involvement
How families deal with safety what can we learn from this?
A team based approach to safety improvement
Developing a brothers keeper mentality and approach
Recording safety concerns what are the features of an
effective process?
Data collection and collation secondary analysis to identify
trends
SESSION 12: DESIGNING YOUR OWN APPROACH TO BBS
Factors associated with successful implementation
Some of the ingredients to build into an approach to BBS
The importance of careful planning
VDrafting out your own ideas and identifying what help you
might need
Activity: working on own or in company groups to draft out
a design and next steps.
