PETITION
UNDERSIGNED PETITIONER, unto this Honorable Judge,
most respectfully states:
1. He is a Filipino citizen, born on 18 th day of July 1988,
with office address at 9/F Filinvest One Building, Northgate
Cyberzone, Alabang-Zapote Road corner Northgate Avenue,

2. He has been admitted to the practice of law in the
Philippines in 2015 under Roll of Attorneys No. 64999.
Attached is a certified true copy of the Certificate of
Admission to the Philippine Bar duly issued by the Supreme
Court of the Philippines marked as Annex A;
3. He has all the qualifications and none of the
disqualifications for the issuance of a commission as Notary
Public and that he has no other commission as Notary Public
in any other jurisdiction;
4. He is of good moral character and has not been
criminally charged, much more convicted, of any crime

involving

moral

turpitude

and

he

has

no

pending

administrative case before the Supreme Courts Office of the

Bar Confidant. Copies of his Certificate of Good Standing
issued by the Bar Confidant and by the Integrated Bar of the
Philippines are herein attached as Annex B and C,
respectively. A copy of Certificate of Good Moral Character
issued by executive officers of Integrated Bar of the
Philippines (IBP) Paranaque, Pasay, Las Pinas, Muntinlupa
Chapter is likewise attached as Annex D;
5. He has paid his IBP annual dues for 2016 and issued
with IBP Lifetime Number 816097 as shown in the IBP receipt
enclosed as Annex E. He has also paid the required
professional tax for the year 2016 as shown in Professional
Tax Receipt No. 1007327 dated January 22, 2016, hereto
attached as Annex F;
6. He has attached three (3) specimen of his official
signature, marked as Annex G and three (3) of his passportsize photographs bearing his name at each of the bottom,
marked as Annex H;
8. Finally, he has paid the required filing fee as
evidenced by the attached receipt marked as Annex I.

PRAYER
WHEREFORE, petitioner respectfully prays that he be
commissioned as Notary Public for the City of Muntinlupa.
