Instructions for Priests:

Option 1: Ideally the priest must himself write the answers on this form,
interviewing the party separately and alone. Before the Pre-Nuptial Investigation,
the priest swears in the marriage applicant by asking him/her to touch the Bible
and to answer this question: Do you swear to tell the truth and nothing but
the truth in answering the following questions? (Ikaw ba ay sumusumpa na

magsasabi ng katotohanan at pawang katotohanan lamang sa pagsagot sa mga

sumusunod na katanungan?).
After the interview proper, with clarificatory questions posed and answers
thereto gathered and written down by the priest, he asks the marriage applicant
to affix his/her signature to the form as indicated.
Option 2: If the priest (or his priest or deacon delegate) is pressed for time and
cannot himself attend to the required form as described above, i.e., by writing
himself the answers of the marriage applicant to the questionnaire during
interview, the couple may nonetheless answer in advance and fill-in the forms
individually prior to the interview with the priest or his designate. During the
interview proper the priest (or his priest or deacon delegate) can verify the
answers and ask for clarificatory questions as usual, and then add the necessary
corrections and/or additional information. Again, just like above (option 1), right
before the Pre-nuptial Investigation proper, the priest swears in the marriage
applicant by asking him/her to touch the Bible and to swear to the truth of the
answers he/she has given in the questionnaire provided him/her: Do you swear
to the truth and nothing but the truth of the answers you have just given to
the questions that were posed before you? (Ikaw ba ay sumusumpa na

katotohanan at pawang katotohanan lamang ang mga sagot na iyong ibinigay

sa mga sumusunod na katanungan?)
After the interview proper, the priest asks the marriage applicant to sign and
affix his/her signature on the questionnaire as indicated, and then finally, affix
his own.
