As a members of Osh Committee in Electrical Engineering Department, you and


your team had identify a few hazard and asses its risk to student and staff that have
to face the hazard in their daily routine of work.
a) At wiring workshop; the 2-level bay, the corridor holders are too low, thats
below hips.
b) A hill step at the main entrance JKE heading upward to wiring & project
workshop was being spotted to be a favorites shortcut route.
c) There were plenty of holes on the cement floor around JKE including at stairs
step, classrooms, along corridor and other location.
d) At the computer laboratory, the floors were using hard-plastic material as
tiles. Problem occurs when times flies by, the corners of each tiles were
smash up (terjongket), and risking student an staff of its sharp end corner.
e) JKEs lecturers were full of lust and passions when it comes to teaching and
learning process. So, theyve been beared their equipment of Teaching &
Learning along their way to classes. Each session of class, and each day a
week. From the factor of ergonomic and the hazard exist along JKE
environment; this is a huge risk of accident and illness in long term.
f) There were few laboratories at JKE that had lack of safety from the position of
workbench. Among with the amount and sizes of these benches, it turns out
to full fill the space of this lab to place this bench. In a safety point of view,
these conditions were not being safe at all.
g) All stairs in department were build not according to its specification. The
steps (anak tangga) were too short. Theres a location where it steps where
to short when you place your foot, it only covers half of your foot. This was
too dangerous.

In group, assess the risk by the table of consequence (Table 1) and severity
(Table 2) given.
Determine on which value is suitable for each of your group case.
Get the Risk Priority Number (RPN) according to Consequence X Severity
formula, using the risk matrix table (Table 3).
Each risk level is determined on Table 4 which your team are needed to
discuss among yourselves, why your case are stated on level that you
choose. Elaborate your findings in a full page report.
