With regard to DGM/TO remarks at NP-1, committees observations are as under:Existing manufacturing practices which could have avoided

the incident:1. After first failure of beta pin in RIBE Make cantilever on date 24.04.16 at location 24/22,

the joint checking of Beta pin by O&M and L&T was launched.
2. As on dated 15.07.2016, when the second failure of beta pin took place at location
32/30, O&M along with contractor (M/s L&T) had checked total . Nos. of Beta pins for
any sign of looseness. Out of these total . Nos. of beta pins were replaced.. However,
due to consecutive failures of Beta pin, it was decided by competent authority for
replacement of Beta pin with RIBE split pin.
Suggestion on maintenance practice: - L&T has now already changed the all Beta pins in
Line-6 with split pins. As per existing maintenance practice, during AOH of catilever (24
months), condition of split pin is to be checked whether its mouth is fully open.( copy
attached).
