within the City of Chicago in the County of Cook and State of Illinois, and pursuant to Section 28-1 et. seq. of the Election Code, do hereby petition
that the following advisory question of public policy be placed on the ballot for submission to the voters of all precincts, 1 through 59 inclusive,
within the 24th Ward for their approval or disapproval, by referendum at the Primary Election to be held on February 5, 2008:

Instructions for Collecting Signatures on the Petition

Circulator Must:
• Be a U.S. Citizen (may reside anywhere, including out of state & do not have to be registered voter) and 18 years or older.
• ONLY ILLINOIS VOTERS FROM THE TERRITORY BOUNDARIES DESCRIBED ON THE PETITION MAY SIGN.
It may help to put Post-It tabs on each sheet indicating which county or city it is -- or use different clipboards
for each territory, writing each county or city on the top of clipboard.
• Personally witness each petitioner signing. You may sign your own petition. Exception for handicapped voters: if a signature
is not legible or an X, please fill in the address and write in small letters, "I witnessed this handicapped voter's signature." Board
of Elections said it will probably be challenged so please get petitioner's phone number and send it along with that petition to us
(do not staple). We will verify the petitioner's signature with their County Clerk.
• Ensure each petitioner has printed his/her name and added their street address (NO P.O. Boxes!), including city and county.
Note: Circulator may write in just the name and address of petitioner.
• Carefully fill out the affidavit section on the bottom of each petition. If you have questions, please call us.
• Before a Notary Public, sign name and have it notarized/stamped. Note: A Notary Public cannot sign as a petitioner on the
same petition they are notarizing.
• If petitioner makes a mistake, just cross out, signer initials mistake & then write/sign on next line. Do not use White Out.
• If you copy and share petitions with friends, please be sure to include a copy of the instructions .

Petition Signer Must:


• Be a REGISTERED VOTER. Circulator: Please ask this first – and ask if they've signed it elsewhere – one time only!
• Print name and address including municipality and county. (Circulator may print the name and address only, if needed.)
• Each voter/petitioner MUST SIGN THEIR OWN PETITION.
A County Clerk can disqualify a signature that looks like a spouse signed for both.
ALL information must be the same as on their current registration at their county courthouse. Signatures have been thrown out due to
petitioners signing their nicknames/middle names or the address did not match the one on their registration!! If a nickname is signed, the
Clerk may ask us to go back and identify the voter!
Some signers also don’t realize their names have been purged from the registration rolls if they haven't voted in several years, so encourage
all petitioners to verify current information with their County Clerk & reregister if needed.

+ The petition sheet must be reproduced on 8 ½ x 11 white paper. Partially filled sheets can be sent in.
+ No staples or any extra MARKS are allowed on petition outside the lines, please!
+ Please verify that each sheet is notarized before sending them in or we will NOT be able to use it.
