INTERVIEW GUIDE (20 points)


Think of this assessment as a guide for you to help your client in a stressful situation. Allow the individual to tell
his/her own stories. Avoid a rigid adherence to sequence. Remember that family means different things to different
culture: Define Family if used. Notice that the questions are open ended; meaning a yes/no response is not
acceptable.

1.

Has any kind of spiritual support been helpful to you in the past? Explain.
Yes. When his parents got divorced, God was the only person that he could turn to.

Throughout the divorce, the client felt extremely lonely. He felt as if he did not have anyone
in this world. However, having God in his life, he felt that he could get through the situation
he was in.
2.

How would you describe God? The client describes God as a higher power who is
watching over everyone. He feels that God is a power who can control what happens in
our daily lives. The client also feels that as much as we try to control our own lives, God
has the ultimate say.

3.

Everyone goes through difficult times. In your experiences, has your relationship with
God been helpful to you?

The clients relationship with God has been very strong throughout his life. During
difficult times such as his grandmothers death or even his parents divorce, the client
describes God as his best friend. Having a relationship with God has taught him how to
be a better person overall.
4.

What does religion mean to you? The client feels that religion is describing your
relationship with God. God is someone that you can turn to during difficult times when
there is no one in the world left for you. He also feels that religion is a sense of comfort
in his life.

How do you handle feeling afraid? The client keeps his feelings of fear to

himself. The only individuals he talks about his feelings of fear with are his parents or
God.
6.

What is your source of strength right now? The clients family can get him through any
stressful situation that comes his way. Another source of strength for the client is his
girlfriend. He feels that she makes his life a little bit easier every day.

7.

How can I help you in carrying out your religious practices or in your relationship with
God? Are there objects or activities that would help you? I could accompany the client to a
church service. I could also point out meaningful verses from the bible, which relate to
the situations that we are going through.

8.

Would you appreciate a visit from your pastor? The client would not like a visit from
his pastor. The client does not enjoying having individual attention from pastors,
priests, etc.

9.

Identify 3 experiences, people, things, etc. that give MEANING to your life. Explain
why.
1) Kidney transplant: This event got the client to believe in God again. He was not

previously religious. After this incident, he definitely felt Gods presence.


2) His grandma: The clients grandma raised 6 kids by herself while not having very
much money. The clients grandma is his inspiration for being a hard worker.
3) His stepdad: His stepdad was there for the client everyday even when his real dad
was not. His step dad is an extremely important part of his life and the client would not be
the person he is today without him.

10.

What activities help you to accomplish your purpose in life? The client volunteers at
the Lions club every Thanksgiving and Christmas, which gives meals to the homeless.
The client also donates blood to help others in need.

11.

Do you have any support people? The clients family and his girlfriend are the
biggest support system in his life. These individuals give him moral support and advice
every single day.

12.

Who is the most important person in your life? The clients Mom is the most
important person in his life. She is always there for him throughout his life and his
mom would never judge him for anything.

13.

Are you involved in the care of another person? The only person the client cares for
is his grandma. His grandma is also his neighbor, so he and his Mom visit her twice a
week.
