Leading Questions to identify Lift Type/Classification

Do you have a ROO Approved Lift Plan (signed by ROO Lifting Authority)
Will the Load be lifted over Live Plant, Pipework or Process Areas? (Process Safety Risk)
Does the ground need to be compacted and/or plated to withstand the ground bearing pressures imposed by
the Outriggers
Does the Lift involve lifting of people
Are the Outriggers within 1.5 x depth the edge of the Excavation to the nearest point of the Outrigger mat
Could the lift be affected by proximity Hazards (Public Roads, Overhead Power Lines etc.)
Is this a Tandem Lift/Multiple Crane Lift or is the Load to be lifted, upended or rotated by two or more powered
Lifting Devices
Does the lift involve the use of Spreader Bars or any other Special Lifting Accessories
Is the lift being conducted in an area with restricted head room or limited boom clearance i.e. less than 1m
Is the Load valuable, irreplaceable, time schedule critical or will result in a significant loss for ROO
Is the Lift non-returnable to the starting location after the lift or classed as demolition
Is the lift a Blind Lift or conducted within a Confined space, Trench or Excavation

Are the Lift characteristics unusual (Heavy, awkward shape, large sail area, difficult to Sling, involves crosshauling or more Complex than normally performed by this type of Lifting Equipment)
Does the Load have an offset Centre of Gravity, Lifting Points below the Centre of Gravity or the potential to
become unstable
Are the Slings to be used at an angle of below 60 degrees from the horizontal
Does the Lift require Technical and/or Engineering input

If you answered no to all of the questions 1-19, then proceed below to produce the Simple/Routine Lift Plan
Completion of the section below shall ensure compliance with Simple/Routine Lift Plan Requirements

Simple/Routine Checklist Questions

The lifting Equipment - Mobile Crane, Side Boom, HIAB etc. has been inspected and approved
with current ROO ATO (Authorised to Operate) Certificate
Rigging & Lifting Accessories - Slings, Wires, and Shackles etc. have been inspected and are in
good condition with colour coding as per ROO Lifting Procedure
All the Safety Devices are in place and functioning with a Daily Checklist completed
Weight of the Load has been identified and confirmed
Lifting area is free from obstructions or Hazards and the Set-Down Area agreed between AA,
Crane Operator and Banksman
The Ground Conditions are suitable to set up the Crane
The Banksman is clearly visible to the Crane Operator and is wearing a HIVIS Vest
All Lifting Crew have attended Lifting Awareness Training and have ROO Safety Passport.
Effective means of Communications is established and tested prior to Lift commencing
Wind Speed has been confirmed as < than 9.8 m/sec

must be attached to the Permit to Work at the Site for the duration of the lift.
