Safety Dated: 06.05.2016 Ganga Ram Agarwal Additional Member Electrical Dear Shri (CEEs of all the Zonal Railways) Sub: Special drive to improve safety - preparedness of Railway system 1. The recent incidence of a side collision between two passenger carrying trains is a matter of serious concern. There have also been few cases of derailment on the railway system on electrical account. 2. __ Last year ie. in 2015-16, 24 cases of signal passing at danger and 3 cases of derailment on electrical account were reported. During current financial year (up to 5" May), 2 cases of SPAD have already taken place. Considering the importance of the issue, a meeting on 27.04.2016 in Railway Board was held to review the incidence of signal passing at danger. Minutes of the meeting incorporating actions taken by Railways have been issued vide this office letter of even no. dated 04.05.2016, 3. However, in view of recent incidence of Safety breach, Railways are advised to launch a special drive on the following important aspects to ensure proper safety:- (i) Signal Passing At Danger (SPAD) : - Proper counselling of running staff + To conduct brake feel & brake continuity as prescribed. The above should be checked by surprise / ambush checks. * Running staff should be counselled not to use walkie-talkies to get information about its signal aspect or other operational instructions from stations staff * Loco pilots should be counselled for not using mobile phones while on run. + Running staff should be counselled through Loco Inspectors/Officers to reduce speed of train in case signal aspect is restrictive and not to presume aspect of next signal + Railways should ensure that signal sighting committees go on line and ensure that there is no obstruction to the visibility of signals during day or night and also prepare printed pocket sized signal location books/cards and distribute the same amongst the running staff. + In case of change in signal location due to doubling or construction works, running staff should be accompanied by Li/safety counsellors and the change of signal location be communicated well in advance through drivers notice book at lobbies. ole * Railways were advised to counsel running staff through Loco Inspectors/Officers to reduce speed of train in case signal aspect is restrictive and not to presume aspect of next signal. + Loco Pilots should be advised to stop the train at adequate distance before foot of signal at ‘On’ position so as to have clear view of signal from cab, subject to clearing of fouling mark. Loco Pilots should apply train brakes by dropping of BP pressure up to 0.5 kg/cm? while waiting for line clear in loop lines for more than 15 minutes. * Calling out of signal aspect loudly by ALP & LP should be monitored closely. All ALPs should be counselled for applying emergency brakes by opening of RS valve in case LP is not vigilant or not reacting according to signal aspect. It was also suggested that ALP & LP should be counselled to call out signal with hand gesture. + Running staff should be advised to avoid the tendency of not to pack their personal belongings while approaching the terminating station. + Railways should give proper Learning Road (LR) to running staff and extend the LR period until staff is well conversant with the sections and confident to work independently. * Strict action should be taken against LPs found isolating VCDs which are otherwise in working condition (ii) Safety : - * Checking of all Safety items of all Electric locomotives and EMUs/MEMUs at sheds /out-pits with emphasis on intactness of load bearing parts, TM assembly, fittings in bogie, brake rigging, under frame & wheel assembly, working of VCD, AWS in EMU's, Headlights, Flasher lights and roof equipments etc. + In addition to above, the progress / implementation of safety items/issues pertaining to Electric locos, mentioned in Annexure-I should be monitored & feedback should be given to Railway Board * Hanging parts of electrical equipments, types of rolling stock viz. electric locomotives,,EMUs, MEMUs & other coaching stock. * Ensuring proper Ultrasonic Testing of axles of all locos, EMUs, TM shaft etc. as per schedule as well as ensuring Ultrasonic Testing is done by Trained supervisor only An action taken report may be sent to Railway Board after 15 days’ drive. RDSO Technical Circular No. ELRS/TC/0047(2000)(Rev-02), dated 19.10.2015 Maintenance of Axle Box Bearings for WAP7&WAG9/WAGSH locos: Sheds to make a survey and document the location of sealed bearings (position wise) on locomotives and give identification marks (a # mark with yellow paint on the axle box cover). Railways should plan the procurement of bearings having provision of re-greasing in- situ to guide lines issued by Railway Board and plan immediate 100% replacement of sealed bearings in WAP7 locos on priority, irrespective of the condition of the bearings.
