2B1.1. Wear respirator

3A1.1. Wear coveralls, nitrile gloves and safety boots. All outer
personal protection should be impervious to liquid or
sludge. Where this is not possible in hot conditions, cotton
overalls may be worn but a proofed apron should be worn
to reduce the contamination.
3A1.2. In cases where there is accidental sludge contact with the
skin, was off at nearby Emergency Shower at once. Eye
wash station shall be used to wash off sludge that gets into
the eyes.
3A1.3. Observe proper / careful handling of sludge.

3B1.1. Tank Cleaners must be physically fit. Compliance to PE


requirements as Tank Cleaner is a must.
3B1.2. Observe buddy-buddy system. Buddy serves as an observer
and rescuer at the same time.
3B1.3. Provide two spare sets of Breathing Apparatus (1 SCBA
with audible alarm & 1 BA from air compressor with a
dedicated operator in place of an audible alarm (authorized
by Engineering).

3B1.4. Agreed communication medium between tank cleaner and


buddy/observer are tagging of the lifeline and sign
language.
3B1.5. The devised & practiced emergency rescue plan shall be
resorted to in any rescue operation.
3B1.6. Work only during daytime for best visibility.

heart.

3B2.1. Wear Breathing Apparatus.

getting inside the mask. A vacuum is created once air
supply to BA is cut-off, this is an indication that face mask
fits well. Do this simple test before each Confined Space
entry where BA is needed..
3B2.3. Provide a good working air compressor complete with a
working air regulator and with leak free fittings, hoses &
hose connections.
3B2.4. Breathing air must be clean and free from contaminants.
The supply method must have a dedicated operator in lieu
of audible alarm requirement (authorized by FFE P.
Ocampo on 13th Dec.03) to ensure enough air in

compressor to support BA in use and the back-up.


3B2.4. Tank cleaners and Rescue personnel must have to undergo
refresher training on the proper use of BA / SCBA.
3B3. Toxic Fumes

3B3.1. Ventilate tank with air blower before and during entry. A
watcher must be in place to take notice of power outages
who advises immediate tank evacuation once electrical
power goes off.
3B3.2. Wear Breathing Apparatus.

3B3.7. There shall be not Confined Space Entry during tanker
receiving operations.
3B3.8. Observe personal hygiene workers must wash and change

into clean clothes before eating.


3B4. Flammable Vapours

3B4.1. Test for flammable vapours before entry. Entry is not


allowed even with Breathing Apparatus if the LEL level is
above 10%. Ventilation would have to continue until LEL is
10% or lower, only then would a Confined Space Entry is
allowed.
3B4.2. Any of the depots authorized Gas Tester (C. S. Subido &
D.B. Tam) shall set-up Gas Tester for continuous
monitoring purposes before work in the morning and
afternoon. Trained contractors personnel (Project
Supervisor Orlando Oliveros)
shall do the continuous monitoring of LEL level.
3B4.3. Any portable electrical equipment to be used in the confined
space must have residual current protection. Test the RCD
with an approved tester before entry.

3B5. Poor Ventilation

3B5.1.1 Activate air blower before and during Confined Space


Entry.

3B5. Spills

3B5.1. Provide canvas matting at work area near exit and entry
point into confined space and at sludge containment area to
avoid ground contamination.
3B5.2. Sludge-containing drums shall be provided with canvas
cover to prevent accumulation of rainwater.
3B5.3. Store sludge containing drums in Weathering Pool.

3B6. Rain & Lighting

3B6.1. Stop work and evacuate tank at the onset of rain and
lightning storm.

3B7. Smoke

3B7.1. Stop work and evacuate tank when smoke gets into work
area. It can suffocate workers when it gets into the tank and
can obscure eye contact between buddy and tank cleaner.

6A. ENVIRONMENTAL
HAZARDS
6A1. Toxic Fumes

6A1.1. Activate air blower to accelerate dissipation of toxic fumes.


6A1.2. De-gassing either by natural or by mechanical means shall
last no less than 24 hours. or until a NIL LEL and an
oxygen level of between 20%-21% is achieved.
6A1.3. Workers near manholes shall wear a respirator.

6B1. Confined Space

6B1.1. Tank Cleaners must be physically fit. Compliance to PE


requirements as Tank Cleaner is a must.
6B1.2. Observe buddy-buddy system. Buddy serves as an observer
and rescuer at the same time.
6B1.3. Provide two spare sets of Breathing Apparatus (1 SCBA
with audible alarm & 1 BA from air compressor with a
dedicated operator in place of an audible alarm (authorized
by Engineering).
6B1.4. Agreed communication medium between tank cleaner and
buddy/observer are tagging of the lifeline and sign
language.
6B1.5. The devised & practiced emergency rescue plan shall be
resorted to in any rescue operation.
6B1.6. Work only during daytime for best visibility.
6B1.7. Identify entry and exit points workers must know by

heart.
6B1.8. Ensure that barricade of work site and warning signs are in
place during and even after working hours.
6B1.9. Confined Space worker must wear Safety Harness with
attached lifeline with the other end manned by respective
buddy.
6B1.10.Workers must not exceed a per entry duration of 15
minutes.
6B1.11. Ensure there are no personnel inside before boxing-in tank.
5B2. Low Level Oxygen
5B2.1. Confined Space entry without BA is allowed only when
there is NIL LEL and oxygen level in tank is between
20%-21%. Beyond these levels, confined space entry shall
be made with BA and coveralls.
5B2.2. Wear BA and overalls to test for flammable vapour and
oxygen level in tank.
5B3. Toxic Fumes

5B3.1. Ventilate tank with air blower before and during entry
5B3.2. Confined Space Entry can be made without BA if LEL is
NIL and oxygen level is between 20%-21%..
5B3.3. Wear nitrile gloves and safety boots and cotton overalls.
5B3.4 Position all engine driven equipment at least 15 meters
from the tank and in such way that exhaust fumes are not
blown into the confined space.
5B3.5. There shall be not Confined Space Entry during tanker
receiving operations.
5B3.6. Observe personal hygiene workers must wash and change
into clean clothes before eating.

5B4. Flammable Vapours

5B4.1. Test for flammable vapours before entry. Entry is not


allowed even with Breathing Apparatus if the LEL level is
above 10%. Ventilation would have to continue until LEL is
10% or lower, only then would a Confined Space Entry is
allowed.
5B4.2. Any of the depots authorized Gas Tester (C. S. Subido &
D.B. Tam) shall set-up Gas Tester for continuous
monitoring purposes before work in the morning and
afternoon. Trained contractors personnel (Project
Supervisor Orlando Oliveros)
shall do the continuous monitoring of LEL level.
5B4.3. Any portable electrical equipment to be used in the confined
space must have residual current protection. Test the RCD
with an approved tester before entry.

5B5. Poor Ventilation

5B5.1. Activate air blower before and during Confined Space


Entry.

5B6. Lighting

5B6.1. Stop work and evacuate tank at the onset lightning storm.

5B7. Smoke

5B7.1. Stop work and evacuate tank when smoke gets into work
area. It can suffocate workers when it gets into the tank and
can obscure eye contact between buddy and tank cleaner.
5B8.1. Wear Safety Boots with a good sole for stabilized traction.
5B8.2. Watch your step.

5B8. Slippery Surface


5C. THERMAL
HAZARD
5C1. Suns Heat

5C1.1. For workers outside tank wear hard hat, long sleeve

trousers, and safety shoes.


5C1.2. For Tank Cleaners workers must not exceed a per entry
duration of 15 minutes at one hour intervals.
5C1.3. Observe rests and breaks.
5C1.4. Provide a cold drinking water at work site.

physically imbalance much like alcohols do.

8.1. Keep tools in tool boxes when not in use


8.2. Keep materials and supplies properly stacked and away from
walkways.
8.3. Clean-up work area before leaving.
