AFFIDAVIT OF CONSENT
I, Gaudencio De Jesus Ocampo, Filipino, of legal age, married
and a resident of Tagudin, Ilocos Sur, having been duly sworn in
accordance with law, hereby depose and state:
1. That I am the biological father of Hannah Kate Managdagdag
Ocampo;
2. That at the time of the birth of my biological daughter, Hannah
Kate Managdag Ocampo, I was not married to Carmen Bayoya
Managdag Blakemore, the child's biological mother;
3. That I am giving my full consent to the adoption of my daughter
Hannah Kate Managdag Ocampo by Carmen Bayoya Managdag
Blakemore and her husband Edward John Blakemore;
4. That I am executing this instrument give my explicit consent to my
biological daughter Hannah Kate Managdag Ocampo being
adopted;
5. That I am executing this affidavit to declare the truth of the
foregoing facts.
