Overseas Request for Quotation (RFQ) for Supplies/Services/Equipment

Peace Corps is a United States Agency with volunteers in all counties of Liberia in the
education sector. People are the main resources we have and keeping out staff healthy,
facilitates the work we do in country.
Peace Corps has staff both in Monrovia and Zwedru and often have staff across the 15
counties monitoring the work of the volunteers
Peace Corps Liberia intends to offer a firm-fixed price contract for Group Medical
Insurance services, Local and off shore, for its staff and dependents.
Interested vendors should submit a quote for the services as described in this RFQ
below:
A.

Statement of Work/Specifications

Provision of medical coverage for 12 months period as follows for a minimum of 35 local
staff at least four dependents each:
Among other things, we hope to receive a coverage that covers:
100% Routine medical examination
100% Outpatient daily hospital visits
100% Hospitalization
100% Pregnancy expenses
100% Optical
100% Routine dental services
Evacuation to neighboring countries for treatment and any other policy benefits.
Quotation should be based on group plan.
We also hope your policy will provide the Peace Corps staff and their dependents the
ability to:
Use specialist hospitals
Use hospitals in most major cities of Liberia
Reimbursements for treatments from hospitals not covered by the scheme
Reimbursements for medications from pharmacy
A list of hospitals and clinics in current agreement/contract with the insurance
company
Outline a strategy/plan of how Peace Corps staff will easily access services at
the proposed medical facilities without hindrances

valid/accurate invoice.
