Convention on 27-30 October 2016 at the Crown Regency Mactan, Cebu City. This activity will
take place under the guidance and supervision of the Accountancy Program from the Divine Word
College of Legazpi.
As parent and/or legal guardian, I remain legally responsible for any personal actions taken by the
above named minor or participant.
I agree on behalf of myself and my son/daughter/ward named herein to release the school, DIVINE
WORD COLLEGE OF LEGAZPI, its faculty chaperone and administrators from and against any and
all claims, demands, actions, complaints, suits or other forms of liability that any of them may sustain
(a) Arising out of my childs failure to comply with policies, rules and guidelines of the event;
(b) Arising out of any damage or injury caused by my child
I also understand that this event may expose my child to some risks and I assume any such risk that
may arise there from. If the child suffers injury or illness while on tour, the undersigned hereby
authorizes any faculty accompanying the child to authorize such medical attention for the child as
may be deemed appropriate. I accept full responsibility for all medical expenses for any injuries that
might occur to my child by reason of his/her participation.

I have carefully read this Waiver and Permission Agreement and understand the terms and
conditions of it and agree to be bound thereby. My child/ward has the undersigneds permission in
the 19th National Mid-Year Convention.
My child/ward will not participate in the 19th National Mid-Year Convention.

STUDENT UNDERTAKING
I, MARLON G. SOLANO of BS ACCOUNTANCY - 5 present myself to join the 19th National MidYear Convention which will be held on 27-30 October 2016 at the Crown Regency Mactan, Cebu
City as part of our school/student and do hereby pledge:
1. That I have read the rules and guidelines made for the event;
2. That I will obey and abide the rules and guidelines made by the Faculty In-Charge for the
event for the protection and safety of all;
3. That I have attended and fully understand the orientation given for the event;
4. That I hereby waived and renounce my rights to all damages, hospitalization, and the like;
5. That I will not hold the school, faculty, and the agency of MUST responsible for any
misfortune, injury, or accident be slight or serious, that may happen in connection with the
activities or requirements. The cause of which will be attributable to my acts of my
disobedience, negligence, and the offense of my heading to advise, warning, precaution,
and safety rules that were read to me by adviser before the start of the activity.
That the content of this waiver was read and explained to me before I affix my name and signature
this day of OCTOBER 22, 2016 freely and voluntarily.
