Indicate the method used to invite the student to the transition meeting: Rolanda was invited to
her transition meeting using picture symbols. She was given the option to choose from various
picture symbols. She was given the option to go or to not go through different picture
representations using a communication board.
Age Appropriate Transition Assessments and Summary
Personal Interests: Rolanda is very interested in music and responds to be alert when music is
provided. She enjoys many different activities when given the choices. She likes to go to the
movies and loves to go to many different places with her family such as the zoo, baseball games,
and even university events.
Training/Education: Rolanda began her education in a self-contained environment when she was
in elementary and middle school. However in high school all of her teachers were pushing he to
be in an inclusive setting for some of the day. She spent most of the day in the inclusion setting
and for two periods of the day she was in a resource classroom.
Employment: Rolanda is not currently employed, however she does have an interest in music
and would love to be able to play music for a radio station. In looking at the future we want to
make sure that we involve her in something that is related to music and the opportunity to listen
to it while working.
Independent Living Skills (Optional): Rolanda needs round the clock care. After high school her
parents are planning on having her in the care of a relative. We want to help her to become more
independent in her abilities to make choices for her self and communicate them in a way that
allows her independence in communication.

from Project SEARCH a transition team here in Tucson at the University of Arizona had

Measurable Post-Secondary Goals
Training/Education Goal: After high school Rolanda will be able to identify five sight words
related to her wants/desires at home or in the community by pointing at the correct picture
matching the word using a pointing stick in her mouth.
Training/Education Connected IEP Annual Goal: Rolanda will increase her sight word
reading by pointing at the correct picture matching the word using a pointing stick in her
mouth by April 29th, 2016.
Employment Goal: After high school Rolanda will attend a transition program in Tucson, AZ
called Project SEARCH. Seeking internship opportunities in the music department.
Employment Connected IEP Annual Goal: Rolanda will be able to increase her
alphabetizing skills by selecting the correct alphabetized set using the authors name to
go first, second, or third in a set using adapted Explain Everything (iPad app) sets by
May 5th, 2016.
Independent Living Goal (When necessary): After high school Rolanda will attend lectures at
the University of Arizona specific to using augmentative communication.
Independent Living Connected IEP Annual Goal: Given a communication board with
wants/desires, foods, and feelings Rolanda will be able to communicate three
communication exchanges between her teachers specific to wants/desires, foods, or
feelings by pointing at the appropriate exchange using a pointing stick in her mouth by
April 30th, 2016.
