Restaurant Technologies offers a choice of medical plan options through Blue


Cross Blue Shield of Minnesota. Each plan includes comprehensive health care
benefits, including coverage for prescription drugs and free preventive care services. This
year, there are no changes to your premiums. New in 2017, you will see reduced office and
urgent care copays for the PPO Plan, and the addition of Value Based medical services and
prescription coverage on both plans for specific conditions.

The Health Savings Account (HSA) is only available if you are enrolled in the Consumer
Driven Health Plan (CDHP). Contributions to an HSA can be used for medical
expenses for you and your dependents, even if those dependents are not covered
under your health insurance plan. Contributions are made pre-tax and earn interest,
just like a regular savings account. *IRS dependent child age limit is 24.
HSA funds are tax-free if the money is used for medical expenses. Health Savings
Account dollars roll over year to year. Contributing to an HSA is a great way to pay
for medical expenses. (However, there are federal limits on how much may be
contributed.) Restaurant Technologies helps you fund your Health Savings Account.

Life Insurace
Life insurance is a necessary way of protecting your family from financial hardship due to an
untimely passing. Accidental Death & Dismemberment (AD&D) insurance is designed to provide a
benefit in the event of accidental death or dismemberment. Restaurant Technologies provides a
flat amount of $50,000 of Basic Life and $50,000 of AD&D Insurance to all eligible employees at no
cost to you.

SUPPLEMENTAL LIFE INSURANCE


Additional life insurance is available for you, your
spouse, and/or your children. This voluntary benefit
is 100% employee paid. Here are the details of the
Voluntary Life Insurance benefit:
Employee: Up to five times your annual salary, with
a maximum of $500,000. (Example: employee annual
salary is $50,000, maximum benefit is $250,000).
Spouse: Up to 100% of the employee-elected amount,
maximum of $500,000. *Spouse Coverage ends age 70.
Child(ren): A flat amount of $1,000 from live birth to age
6 months; or increments of $2,000 up to a maximum of
$10,000 for ages 6 months to 26 years old. Dependent
Child monthly life rate premium is $0.30 per $1,000

unexpected emergencies. Restaurant Technologies offers additional benefits
through UNUM. It can help protect your finances from the uncertainty of the
future and give you peace of mind. The following benefits are available to eligible
employees on a voluntary basis, and are paid for by the employee.
Whole Life: Whole Life Insurance can pay money to your loved ones if you die. But it also offers additional value: a
living benefit. If you are diagnosed with a terminal illness and have a life expectancy of one year or less, you can request
some or all of the death benefit while you are living. Whole Life Insurance premiums wont increase with age, and your
policy can build cash value over time. You can use the cash value later in life to buy a smaller, paid-up policy with no
more premiums due.
Accident: With the high cost of medical care today, a trip down the stairs can hurt your bank account as much as your
body. Accident Insurance can pay you money based on the injury and the treatment you receive, whether its a simple
sprain or something more serious like a broken bone. Your plan can pay benefits for emergency room treatment,
stitches, crutches, injury-related surgery and a list of other accident-related expenses. The money is paid directly to you
and you decide how to spend it. You can also purchase coverage for your spouse and dependent children.
Critical Illness: Whats a critical illness? Heart attack and stroke are a couple common examples. This coverage also
includes serious conditions like permanent paralysis, and some policies can provide coverage for cancer. Treatment
for these conditions can be very expensive, so Critical Illness Insurance can help by paying a lump sum directly to
you at the first diagnosis of a covered condition. You decide how to spend the money, and you can also purchase
coverage for your spouse. Dependent children are automatically covered at 25% of your benefit amount. ($75 wellness
reimbursement per calendar year for each insured)

consider. This program is in place to assist our employees and families with child care, senior care,

The Cigna EAP provides a 30-minute free phone consultation with a qualified specialist on issues
such as tax preparation, debt counseling and planning for retirement. The program also provides

401(k) Retirement Savings Plan


Restaurant Technologies wants to help you plan for your retirement with a 401(k) retirement savings plan.
Every dollar counts when you are saving for retirement regardless of where you are in the journey. Starting at a young
age, you can steadily build wealth by taking advantage of tax-deferred growth over a longer period of time. If you are 50
or older, you can take advantage of making a catch-up contribution which allows you to put away an additional $6,000 into
your 401(k) plan.
For example, a contribution of $100 a month to a 401(k) plan can grow to $52,093 in 20 years, (based on a 7% rate of
return). So, a total contribution of $24,000 turns into $52,093. Returns may vary, but its easy to see how your money
grows over time.

BONUS POTENTIAL
Employees are eligible for a number of bonus opportunities based on your job
function. All employees can receive a $2,000 bonus for each employee
referral that is hired and employed with Restaurant Technologies for

TUITION REIMBURSEMENT
Restaurant Technologies recognizes that the skills and knowledge of our

provide up to $2,000 per year for educational assistance to all eligible regular
