I, JEROME BAGANGAN LLANETA, of legal age, Filipino

citizen, married to Laura Wheeler Llaneta, and with permanent
residence address at No 11, Lower Brookside, Baguio City after
having been sworn to in accordance with law do hereby depose
and state:

A. I am Jerome Bagangan Llaneta, a Filipino citizen,
married to Laura Wheeler Llaneta, of legal age and with
permanent residence address at No. 11 Lower Brookside,
Baguio City .
2. Q. You said your name is Jerome Bagangan Llaneta, how
are you related to Jerome Bagangan Llaneta, one of the
petitioners in the above-entitled adoption?
A. I am the same person.
3. Q. You said you are married to Laura Wheeler Llaneta,
when and where did you marry Laura Llaneta?
A. On September 3, 1994 in New Jersey.
4. Q. What proof do you have if any to show that indeed you
are married to Laura Wheeler Llaneta?
A. I have a copy of our marriage contract.
5. Q. How many children do you have with your wife?
A. We have two girls namely Amanda Kirstern Llaneta
and Melissa Ann Llaneta.
6. Q. When and where were they born?
A. Melissa Ann was

born on March 9, 1990 and

Amanda Kirsten was born on June 6, 1998 both at New

Jersey

7. Q. How old is your first child?

A. She is already 22 years old.

8. Q. What about your second child?

A. She is already 14 years old.
9. Q. What is your present occupation?
A. I am a retired Staff Sergeant of the United States
Army.
10. Q. What about your wife, where is she employed if she
is?
A. She is working as a nurse in New York.
11. Q. You said you are one of the petitioners in the aboveentitled case, what is the petition all about?
A. Its about our desire to adopt Lester Dan Padawan.
12. Q. Who is Lester Dan Padawan?
A. He is the son of the brother of my sister-in-law,
Mary Jane Padawan Llaneta.
13. Q. Who is the husband of Mary Jane Llaneta?
A. My brother, Louie.
14. Q. How is your sister-in-law related to Lester Dan
Padawan?
A. Mary Jane Llaneta is the sister of Nelson Padawan,
father of Lester Dan Padawan.
15. Q. Why do you want to adopt Lester Dan?

A. I have always wanted to have a son. I always

wanted a child to carry my name even if he gets married. I
also felt that having a son in my family would make my
family complete. Thus when I was informed by my brother
Louie that the parents of Lester Dan were willing to have
him adopted considering that they cannot provide for his
needs, I immediately informed them of my desire with my
wife and children to have Lester Dan as a member of our
family.
16. Q.

What was the reaction of your biological children

when you first informed them of your plan to adopt Lester Dan?
A. They were very happy and were agreeable. They
said they will cooperate and they signed documents
showing their conformity to the adoption. They were very
eager to have Lester Dan join us and were already having
plans of how to spend family time with him if given the
chance.
17. Q. With your present employment, how much do you
earn?
A. Around 8,000 dollars a month including the other
benefits.
18. Q. What proof do you have if any with the income and
benefits you are receiving?

A. I have a letter from the Department of Veterans

Affairs and my income tax return which I filed jointly with
my wife.
19. How about your wife, how much does he earn?
A. An average of 4,000 dollars gross a month.
20. Q. With your income, would it be enough if one child is
added to your family.
A. Definitely it will be enough. As a retired Staff
Sergeant of the United States Army, my children are
already guaranteed with their medical and educational
needs. Once the Honorable Court grants our petition for
adoption, the benefits of my biological children will also
be enjoyed by Lester Dan.

21. Q. How old is the child, Lester Dan Padawan?

A. He is already 1 year old and 6 months.
22. Q. How is your relationship with him?
A. It is very good. I do not feel that he is from a
different family considering that I know his father, he
being a brother of my sister-in-law and we are close. I
communicate with Lester Dan via skype. From the time he
was born, he was already taken cared of by my brother
Louie and his family thus there was no problem in
communicating

with

him.

When

we

arrived

in

the

Philippines, he slept with us, dined with us and always

came with us wherever we went. We are having great
bonding moments.
23. Q. Who takes care of Lester Dan in your absence?
A. My brother, Louis Llaneta and my sister-in-law
Maryjane Padawan Llaneta.
24. Q. Who provides for the needs of Lester Dan?
A. Me and my wife. We send money through my
brother Louie and sent some stuffs for him.
25. Q. Since when have you been providing for the needs of
Lester Dan?
A. Since he was eight (8) months old.
26. Q. Do you have a house of your own or you are renting?
A. We have a house of our own.

27. Q. How else do you plan to take care of Lester Dan if the
Honorable Court will grant your petition for adoption?
A. We will

take him to states for him to the United

States to study and provide all his needs the best way we
can. My first child is very much willing to help in any way
she can in order to provide a comfortable and decent life
for Lester Dan if the Honorable Court will grant our
petition.
28. Q. Have you been inflicted of an incurable disease?

A. No. I

never had except for my past history of

hypertension, hyperlipidimia, obstructive sleep apnea and

PTSD which are all manageable and controllable.
29. Q. What about any medical ailment that might affect
your ability to take care of Lester Dan?
A. None. I am emotionally and physically able to care
and provide for the needs of Lester Dan except my past
medical history. I was issued a medical certificate that my
medical problems are well controlled.
30. Q. What proof do you have if any that indeed you had
such past medical history but are already controllable?
A. I was issued a certification by JMH Medical Practice
31. Q. What is your religion?
A.I am a Roman Catholic.
32. Q. What about your wife?
A. She is a Lutheran.

33. Q. What negative effect did your different religious

membership had if any in your married life?
A. None. After all, I worship the same God with my
wife. Our different religious membership was never an
issue to us.
34 Q. Have you been convicted of a crime?
A. No.

35. Q. What documents would you show to prove that you

were never convicted of a crime involving moral turpitude?
A. I have my court clearance, my NBI clearance, my
police clearance and barangay clearance all secured here
in Baguio City. I also have Fingerprint Response from the
New York State Division of Criminal Justice Services from
New York showing that I do not have available prior NYS
information. I also have a memorandum from the new York
State Division of Criminal Justice Services stating that I
have no criminal or non-criminal case history information
on file with their office. A letter was also sent to me from
the State of New York, Division of Criminal Justice Services
stating that I have no criminal history on file with their
office.
36. Q. With the petition that you filed with your wife, what is
your prayer with the court?
A. It is my earnest prayer that the Honorable Court
grant the joint petition for adoption I filed with my
wife so that we can have Lester Dan as our son and
for him to carry our names and enjoy a better life
with us.

37. Q. If the Honorable Court will grant your petition, how

would you show your love and affection to Lester Dan?

A. I would love to bring him to school as anyway there

are available good schools in New York. I am willing to
work with my wife and children is seeing to it that Lester
Dan grows as a God fearing person and for him to be able
to finish his studies. I would love to go fishing with him
and to do such other stuffs that a father and son will do to
enjoy together.
38. I have executed the foregoing to attest to the truth of all
the allegations herein and to support the petition I filed with my
wife to adopt Lester Dan Nillo Padawan and for all legal intents
and purposes it may serve.
