The Case Report of Mrs.

N with Uncontrolled Type 2 Diabetes Mellitus


with Diabetic Neuropathy and Retinopathy in The Working Area of the
Puskesmas Teluk Naga, KecamatanTeluk Naga, Kabupaten
Tangerang, Propinsi Banten

Abstract
Background : This case presents a woman with numbness in the hands and feet.
Numbness in hands and feet in adults is most often caused by disorders of the
peripheral nerves, but in this woman is a complication of type 2 diabtes mellitus.
Case : A 44-year-old woman came to the Puskesmas complaining of numbness in the
hands and feet since 1 week ago, does not spread, no pain and shown continuously
throughout the day. She also complaints her vision becomes more blurred since the last
1 month. Patients regularly taking medication since she has Diabtes mellitus type 2
about 8 years ago but her blood sugar levels remain high.
The clinical condition was good with the examination of sensibility resulted
hypoasthesia on both hands and feet. Random blood sugar test obtained 454 mg / dL.
Final diagnosis was diabetic neuropathy and retinopathy caused by complications of
uncontrolled type 2 diabetes mellitus.
Glibenclamide, metformin and vitamin B12 are given, also the eating menu and
exercise was provided as the management. The results of the evaluation of patients
showed improvement after the management of the patient's clinical condition..
Discussion : By using the Mandala of Health and approach to family medicine, found
that the causes of complications of type 2 diabetes mellitus in Mrs. N is the wrong
lifestyle and lack of knowledge about type 2 diabetes mellitus. After the intervention, the
complaint such as numbness and blurred vision decreases, Mrs. N and family
knowledge about diabetes mellitus increased and blood sugar level has improved.
Conclusion : The intervention have good results, complaints of numbness in the hands
and feet and also blurred vision decreases and blood sugar levels decreases at the end
of the intervention. Expected interventions can improve patient knowledge and their
families about type 2 diabetes mellitus and prevent further complications.
