1. VISION Quality and relevant education for its clientele to be globally competitive,
culture-sensitive and morally responsive human resources for sustainable development.
MISSION Help accelerate socio-economic development, promote harmony among
diverse communities and improve quality of life through instruction, research, extension
and resource generation in Southern Mindanao. The Code of the University of Southern
Mindanao states the following general objectives of the university as follows : 1. To
provide programs of instruction and professional training primarily in the fields of
science and technology, especially in agriculture and industrial fields in order to produce
graduates who are both locally and globally competitive and marketable; 2. To promote
advanced studies, research and extension services and progressive leadership in all fields
of agriculture, including agri-business, food and nutrition, fishery, forestry, animal and
veterinary sciences, engineering, industrial education, and other areas of disciplines
needed for the development of Mindanao; 3. To develop courses at the graduate level
along its fields of specialization to respond to the needs of development workers in the
region; 4. To provide non-formal education and undertake vigorous extension and
research programs in food production, nutrition, health, and sports development; and,
5. To offer scholarship and/or part-time job opportunities to deserving students from
low-income families. 2. The core areas of the University are Instruction, Research,
Extension and Resource Generation. At present it holds a center of excellence and
development status in Agricultural Education, Veterinary Medicine and Biological
Sciences. 3. In the coming years, the University endeavors to become a university
focused on research and extension activities in various disciplines. 4. The University
desires to develop excellence in conducting researches in the field of agriculture and its
allied sciences. Also it endeavors to strengthen its research capacity in the field of social
sciences and humanities. 5. The University serves as the center of different disciplines in

the region. In fact, it houses several research centers which caters not only to the
regional needs but also to the national needs of the country. 6. At present the University
is actively involved in extending its capacity and resources to the development of
different processes and paradigms in the community most specifically on the agricultural
needs of the community and governance of the local government units. Several projects
and programs were already fostered between the University and the nearby LGUs to
cater the specific needs of the latter. 1. The University welcomes all support for faculty
development from different agencies both local and international. In the past, several
faculty were already sent abroad for professional development and this continuos to be
so even at present. Local scholarships also enrich the means of the university to send the
faculty for masteral or doctoral studies. Scholarship giving bodies such as CHED, DOST,
DA-BAR and others are constantly providing funds for the studies of the USM Faculty.
For those who have not availed of such scholarships, there is a local scheme in the
University, though very meager, which provides assistance for those who are studying
for masteral and doctoral degrees and for those who are writing their thesis and
dissertation. 2. The age bracket of the faculty in the university is in between 23 to 50
years old with a considerable number who are over 50 years old. The faculty with
doctoral degree are less than 50% of the population. 3. Several preparations were made
most significant of which is the offering of the Senior High School which will serve as
buffer for the large loss of faculty workload. 4. The sending of the faculty members to
graduate studies significally augments the large loss of faculty workload. Many of those
who were sent to graduate studies were those who have not yet acquired a graduate
degree and have lost their faculty workload due to the very small number of first year
enrollee. 5. The University encourages the faculty to conduct research and extension
activities. The introduction of the IRSE by the CHED is also a good avenue for the faculty
to enhance their competencies during the transition period. 6. The university observed
balance in sending the teaching and non-teaching personnel for scholarship through
proportionate allocation. The number of the faculty and staff sent for the scholarship is
in proportionate to the number of teaching and non-teaching personnel of the
University. 7. Yes. The faculty workload, the present needs of the University in terms of
expertise and the future projection as to what will be needed were factors considered in
the nomination process.
At present, a considerable number of our faculty members, some of them are in
contract of service status, do not possess the required graduate degrees necessary to
teach the subject matters they are teaching. Also, many of our senior faculty members
who are already experts in their field are already qualified to retire in the next five years.
In the College of Agriculture for example, 23 of the present 28 doctors degree holders
are qualified to retire or have already retired by 2021. And this scenario is true to almost
all the colleges in the University. These concerns among others threatens the quality of
education the University provides to its clientele in South Central Mindanao. As such, the
University through this scholarship program hereby nominates faculty members and
staff from the different units in the university who are threatened by the challenges
previously mentioned. Most of the nominee for the Doctoral program are faculty
members who have already served several years in the university however they still lack
the appropriate doctoral degree necessary for the subject matter they presently teach.
The nominees who are in contract of service status are those whose potentials are
considered to be very salient in realizing the long term goals and objectives of the
university.
Yes, everything are of quality and completeness. The University ensures that all of them
are highly competent to successfully finish their chosen programs. The nominees were

duly nominated by their immediate supervisors and had passed the necessary criteria as
provided in several issuances of the Commission on Higher Education

1) The proposed degree of the nominee will also help the department deliver and render its services effectively
and efficiently since the Department of Mathematics and Statistics offers Doctor of Education major in
Mathematics, Master of Science in Teaching Mathematics in coordination with the Graduate School that cater
teachers teaching Mathematics in the Elementary, Secondary and Tertiary level. It also caters the major subjects
of Bachelor of Secondary Education Major in Mathematics students of the College of Education producing
globally competitive and competent graduates with enough and appropriate knowledge, skills and qualities
adequate for employment contributing to regional and national development. As such, being an active faculty in
the Department, there is a dire need to update and to vertically align his field of specialization with the new
trends, issues and topics about the discipline.
2)In pursuit to universitys VGMO to produce globally competitive clientele, the need for globally competitive
teachers is very timely for them to keep abreast the demand of globalization. Hence, pursuing the degree is
essential tool for strengthening the Universitys Mathematics Education to offer quality Education that could
compete in the regional, national and even in the international arena.
3)The nominee is with the Department of Mathematics and Statistics for more than 4 years. The applicant has
excellently and conscientiously performed his duties and responsibilities as faculty of the Department of
Mathematics and Statistics. He was part of students researches in the university as a statistician and was also
an adviser of the University of Mathematics Society. He is equipped with skills and knowledge that are needed
in performing his tasks as faculty. In addition, as faculty, he is very prompt in submitting all documents and
papers as mandated in his plantilla position. Thus, by his desire to continue further studies, he will become a

1) In pursuit to universitys VGMO to produce globally competitive clienteles, the need for globally competitive
teachers is very timely for them to keep abreast the demand of globalization. Hence, pursuing the PhD in
Mathematics degree is essential tool for strengthening the Universitys Mathematics and Mathematics
Education to offer quality Education that could compete in the regional, national and even in the international
arena.
2)By his desire to continue further studies, he will become a very potential asset especially in attaining
academic excellence specially in handling major subjects like Real Analysis, Abstract Algebra and Topology. The
teaching force of the Mathematics department, Graduate School and of the university will be strengthened
towards the demand of ASEAN integration and global competition. Further, the proposed degree of the
nominee will also help the department deliver and render its services effectively and efficiently since the
Department of Mathematics will plan to offer Bachelor of Science in Applied Mathematics, and currently
offering Master of Science in Teaching Mathematics and Doctor of Education Major in Mathematics Education
in coordination with the Graduate School that cater mathematics teachers in the Elementary, Secondary and
also in Tertiary level. Being an active faculty in the Department, there is really a need to update and to vertically
align his field of specialization.
3)The nominee is a graduate of BS Applied Mathematics from the University of the Philippines and has the
passion to teaching and learning teaching pedagogies hence he enrolled to MS in Teaching Mathematics.
Professionally, the applicant has excellently and responsibly performed his duties and responsibilities as Faculty
of the Department of Mathematics and no record of misconduct and complaints from his students and
colleagues that tainted his reputation. In research, he has conducted and presented researches locally and
internationally in mathematics and mathematics education. To date he has published four research
outputsthree in local journals and one in an international level. He also rendered extension and production
services in and outside the University. He has also advised and assisted undergraduate theses in BS Education in
Mathematics. By these, it reflects his passion and diligence in teaching and in research. Hence, the nominee
exhibited a strong potential to the improvement of USM specifically in the fields of mathematics and
mathematics education. The nominee has notable contributions. In research, he has four publications including
one from international level in the Proceedings of the 6th International Conferences in Sciences and
Mathematics Education. In extension, he is actively participating in the extension programs of the College of
Arts and Sciences. As an adviser of the University Mathematics Society, he spearheaded a free tutorial in
mathematics to USM students before midterm and final examinations.

1) The nominee is a graduate of Bachelor of Science in Agriculture major in Plant Breeding and Genetics in the
University of Southern Mindanao and he is also a Licensed Agriculturist. He serves in the College of Agriculture,
Plant Breeding and Genetics Department, as Instructor I for almost a year. He is teaching Genetics,
Biotechnology, and Crop Science, in which it enhances his communication skills, writing capacity, teaching
capability and information dissemination. Based on his performance he has the ability to serve the students
especially the University and it was proven during the staff evaluation of the University.
2)The opportunity to earn his masters degree could help him uplift the College of agriculture in the field of
Research and extension programs. As a teacher, given that the nominee had completed his BS degree in
Agriculture and passed the Agriculturist Licensure examination there are bigger chances for the nominee to
succeed as he has all the requirements of being a leader, researcher, and a teacher. He can replace those
professors who are about to retire and be a proponent in conducting various researches in the field of Plant
Breeding and Genetics.

1) The nominee is currently the Head of the Nursing program in the University of Southern Mindanao and has
worked for almost 10 years cumulatively since 2006. Having propelled nursing education and research in the
university to quality standards, he has always thought of the stakeholders satisfaction of his service while
strengthening problem solving skills and forming innovative solutions. It is high time that he has to earn his
doctorate degree program in nursing to be able to enhance further his leadership and management skills upon
return to the University Improved Nursing Instruction and research capabilities.
2)By the nominees desire to earn his PhD in Nursing, he could really be our asset having set an exemplary
performance in his teaching and research works. The applicant has always been at the top of his craft and
successful in both startup and turnaround management roles able to provide to its clientele the quality of
service. Having been consistently rated very satisfactory and at times excellent in his performance evaluation, I
am confident that he could deliver his best towards achieving his dream to become a full-fledged PhD in Nursing
degree holder. By earning the said degree ,the nominee will be able to develop a sense leadership in order to
uplift the quality of education in nursing and the nursing profession itself
3)The nominee really pays attention to details in whatever he does because he wants to make sure that
everything goes well in his work. He is somewhat like a perfectionist. His interpersonal skills as well are
impressive and commendable because he could really get along well with others. He is very passionate about
his craft and his work ethics is truly admirable. As his immediate superior being the Dean of the College of
Health Sciences in USM, I have the trust and confidence in him.

1) The need for doctorate degree is a response to the statement of the mission of the university that quality and
relevant education for its clientele, the nominee chosen degree program is relevant and vertically aligned to the
program of the department as well as the university. The PhD. In Technological Education Management is
appropriate degree to become an asset of the university to mold the students in both undergraduate and
graduate program to be globally competitive human resources. It is high time for him to benchmark from the
institution where they are in to improve the current curricular offering of the department. In the challenge of
the university to be a research leading institution, current gaps that only few of the faculty engage in research
and target to involved all faculty in research activity.
2)The nominee is effective teacher in electrical technology through his evaluation, ability in communication
skills and actual manipulation ability. As an immediate superior I assure that he will abide with the specific
provision as stipulated in the return service agreement to finish his doctorate degree after three years and
reentry plan as stated in the contract, render services in the university until retirement.
3)The nominee is a graduate of Bachelor of Science in Industrial Education Major in Electrical Technology, in
University of Southern Mindanao. He pursued his Master of Professional Studies in Development Education in
the University of Southern Mindanao, and he is a Registered Master Electrician, and NC II Building Wiring
Electrician. He enrolled another masters Degree at University of Southeastern Philippines taking Master in
Industrial technology, completed academic requirements. He served the university for 27 years as electrical
instructor. Based in my observation as a superior, He shown professionalism, responsible, resourceful, and
reliable with desirable attitudes and traits.

1) The nominee will enroll PhD in Technological Education Management at the Don Mariano Marcos Memorial
State University at San Fernando city, La Union. He can help the university in its quest to become a research
university in this part of the country because when the nominee reenter in the university, he will conduct
researches and extension activities relevant to technological education and various trades and a competent
faculty in the department
2)The nominee is as of now is 44 years old, after three (3) years he will come back in the university and in
particular in the department to perform his duties as a new breed and competent faculty and at the same time
conduct shall researches and extension activities that can help the university in its quest to become a level 4
and a research university. He will also collaborate with the other faculty members in the department and in the
university to develop curriculum the best cater the needs of its clientele especially on the implementation of
the K to 12 programs

1) 1. The nominee is teaching or holding a subjects in line with his degree he graduated, as universitys vision

degree (MTTE) in line with his undergraduate degree which is (BSIE) major in automotive technology and

graduates. 1.1. In the challenge of the university to be the leading research higher educational institution and as
the mission statement to; Help accelerate socio-economic development, promote harmony among diverse
communities and improve quality of life through instruction, research, extension and resource generation in
Southern Mindanao .current gaps that only few of the faculty members are engaged in research and target to
involve at least more than 50% of its faculty in research is significantly big. With the nominees chosen degree,
we are optimistic that he will be involved in research activities hence by now he is our undergraduate research
coordinator and submit significant research proposals to the university 1.2. His advanced studies is also in

2)The nominee is a very effective instructor in electronics technology through his observed communication skills
and handson capabilities. His performance based on the conducted evaluation of peers, supervisor and the
students of the University had resulted to a very satisfactory performance consistently. I am assured as his
immediate supervisor that he will abide with specific provisions stipulated in the return service agreement to
finish his degree after 3 years, and reentry plan with 1:1 ratio of service to render in the university and until his
retirement
3)He had shown professionalism and enthusiasm in his present designations as instructor, expertise in the field
of electronics where in fact he is our current technical operations supervisor of DXVL-FM run by the university.
He is always tapped as committee chairs for big events in the university requiring audio electronics expertise.

University of Southern Mindanao and he is a licensed Electronics Technician (ECT). He pursued his Master of
Arts in Teaching Industrial Arts in the USM Graduate School and finished it in 2013. He serves as an instructor in
Electronics Technology in the Department of Industrial Technology (DIT) of USM as Instructor I for almost 10
years and rising to its present position as Assistant Professor I serving the department for 14 years. 3.2 Based on
