WAIVER & RELEASE FORM
I, the undersigned, do hereby wilfully acknowledge that my signature below attests to my
understanding and agreement, with regard to the voluntary participation of my ward in the Youth
Champions League 2016, as follows:
That my ward will conduct himself / herself in an honourable, fair and sportive manner, and will not
/ result in any type of harm to the YCL;

I understand and agree that in the event my ward displays conduct that the YCL considers violent,
unsportsmanlike or harmful to YCL or the other participants, on or off the field, my ward may be
dismissed from participation. I agree to pay for any and all damages to any property or indemnities
caused by my ward wilfully, negligently, or otherwise;
I understand that football is a physical, contact sport that involves the risk of injury. I assume all risks
and hazards associated with my wards participation in the sport. My ward is in proper physical
condition to participate in football practices and games, and does not suffer from any medical
condition or existing injury or physical disability that may be aggravated by his/her participation. In
the event that any such medical condition, injury or disabilities were to occur, I will inform the YCL of
the same. I further understand and acknowledge that the risk involved in the course of playing
football may involve loss or damage to my ward or my property, the risk of death, or any other
unforeseen consequences, including those which may be due to the unavailability of immediate
emergency medical care. I understand that it is my responsibility to ensure that my ward will wear
shin guards, properly-fitted and appropriate shoes, and other protective equipment (e.g.,
mouth-pieces), as provided by the standard rules requisite of a competitive football game, to all
events;
I understand and acknowledge that in the event of any injury to my ward during the course of a
football match, including practice games and competitive matches, I am responsible for all medical
expenses arising from the same. I further acknowledge that all costs arising out of travelling to and
from football events, in respect of my ward, will be borne by me. Participation assumes consent.
I authorize my/my wards photograph, picture or likeness, and voice to appear in any documentary,
promotion (including advertising), television, video, or radio coverage of the YCL, without
compensation
I hereby release, waive liability, discharge, hold harmless, indemnify, and covenant not to sue, the
YCL, their associated directors, administrators, officers, managers, employees, coaches, trainers,
volunteers, sponsors and advertisers, and other agents, estates or executors, in respect of any and
all liability incurred in the conduct of, and my wards participation in, YCLs football programmes.
I further acknowledge that this is applicable to owners, lessors, and lessees of premises,
municipalities, government agencies, successors, heirs, and assigns.
As the parent and natural guardian or legal guardian of the participant, I hereby agree to the
foregoing Waiver of Liability and Release for, and on behalf of, the participant (player/minor) named
above. I hereby bind myself, the minor, and all other assigns to the terms of the Waiver of Liability
and Release. I represent and certify that I have the legal capacity and the authority to act for, and on
behalf of, the minor in the execution of this Waiver of Liability and Release.
