1 The cost of the event will be 63,000.00. it is a full service including
the decoration, meals, labor and materials.
2 Ms. Ella Raye Velasco will be totally responsible for this assignment
from our side. She may be contacted for any clarifications required if

3 We require 50% advance in the form of cash or post-dated cheque
before the work date and the balanced before the event day.

TERMS AND CONDITION

2Es Event Manage Product Launch Event which will be held on December 10, 2016
from the hours of 02:00 pm to 06:00 pm in The Grand terrace will pay the total
amount of 63,000.00
for the entire service provided. One half of the total expenses 31,500.00 is to be
paid upon completion of this contract as a deposit to hold the date, (deposit is not
refundable) and the balance of the total amount to be paid before the event.

up and breakdown

PARTICIPANT AGREEMENT, RELEASE AND ASSUMPTION OF RISK

In consideration of the acceptance and participation in the services of 2Es
Event Management, their agents, owners, officers, volunteers, participants,
employees, and all other persons or entities acting in any capacity on their behalf
(hereinafter collectively as 2Es Event Management), I hereby release, indemnify,
hold harmless, and discharge Snazzy Events Planning Services from any all liability
for injury or harm that I or anyone might sustain and I hereby release, hold harmless
and discharge 2Es Event Management from any all claims, demands, or causes of

action, which are in any away connected to my participation in this activity or use of
2Es Event Management equipment to the extent allowable under State and Federal
Laws.
I. I acknowledge that my participation entails known and unanticipated risks that
could result in injury or damage to myself, property, or to third parties. I understand
that such risks simply cannot be eliminated without jeopardizing the essential
qualities of the activity.
THE RISKS INCLUDE, AMONG OTHER THINGS: FALLING OFF THE STAGE RESULTING IN
MUSCOLO SKELETAL INJURIES INCLUDING HEAD, NECK, BACK, HAND, AND ARM
INJURIES.
Furthermore, 2Es Event Management employees have difficult jobs to perform.
They seek safety, but they are not infallible. They might be unaware of a
participants fitness or abilities. They may give incomplete warnings or instructions,
and the equipment being used might malfunction.
2.I expressly agree and promise to accept and assume all the risks existing in this
activity. My participation in this activity is purely voluntary, and I elect to participate
in spite of the risks.
3.I hereby voluntarily release, forever discharge, and agree to indemnify and hold
harmless 2Es Event Management from any and ALL claims, demands, or causes of
action, which are in any way connected with my participation in this activity or my
use of 2Es Event Management equipment or facilities, including any such claims
which allege negligent acts or emissions of 2Es Event Management
Should 2Es Event Management or anyone acting on their behalf, be required to
incur attorneys fees and costs to enforce this agreements, I agree to indemnify and
hold them harmless for all such fees and costs.
5.I certify that I have adequate insurance to cover any injury or damage I may
cause or suffer while participating, or else I agree to bear the costs of such injury or
damage myself. I further certify that I am willing to assume the risk of any medical
or physical condition I may have.
6.I agree that if any portion of this agreement is found to be void or unenforceable,
the remaining portions shall remain in full force and effect.
By signing this agreement, I acknowledge that if anyone is hurt or property is
damaged during my participation in this activity, I may be found by a court or law to
have waived my right to maintain a lawsuit against JEE&CI on the basis of any claim
from which I have released them herein.
