REPORT OF DEATH

When an American citizen dies in the Philippines, the U.S. Embassy is tasked with the responsibility of
preparing the Report of Death of an American Citizen Abroad. After we have completed the Report of
Death, we will send the next-of-kin (NOK), twenty certified copies of this document. The NOK will find
this report of assistance in dealing with insurance companies, Social Security Administration, the
Department of Veterans Affairs, or in any situation in which he/she will need proof of death. To assist us,
please submit the following:
X deceaseds original US passport (s)
X deceaseds original certificate of naturalization, if applicable/available
X original or original certified copy of the duly registered local death certificate with registry number
X original of the result of the autopsy, if applicable
X completed blank DS-2060 (as attached) Please complete highlighted sections.
X a copy of NOKs valid passport or other valid ID
X original or certified copy(ies) of document(s) proving relationship of the NOK to the deceased:
surviving spouse to submit:
marriage certificate, original or certified copies from NSO
X surviving son/daughter (IF deceased: widow/er/divorcee) to submit:
X his/her birth certificate, original or certified copies from NSO
death certificate of deceaseds spouse (mothers/fathers death certificate), if applicable
X daughters marriage contract, if applicable
divorce decree
surviving parent (deceased: single/widow/er/no child/divorcee)
deceaseds birth certificate
surviving sister/brother
deceaseds birth certificate
NOKs birth certificate
NOKs marriage contract, if applicable
parents death certificates

Photocopies (along with the originals/certified copies) of the required original/certified copy of
documents listed above.
Originals will be returned except the death certificate and the DS 2060 questionnaire form.
Please bring the required documents to Window ACS - 1 anytime between 7:30 11:00 a.m. daily,
Mondays through Fridays, except during Philippine and U.S. holidays and the 1 st Wednesday of every
month.
