Hazard Study Methodology

4.1

PURPOSE
To check that the project has been constructed as the design intended, and that actions from previous
Hazard Studies have been completed and implemented in the design and installation.
To check that the operating instructions and emergency procedures comply with any requirements
identified by previous Hazard Studies and are satisfactory for safe operation.
Key aspects include confirmation that:

(a)

Actions from earlier Hazard Studies have been completed either as agreed or that any
changes to the actions are justified.

(b)

All actions and changes have been documented.

(c)

Changes made following Hazard Study 3 have been studied and recorded.

(d)

A detailed occupational health assessment is available.

(e)

Operating instructions are available for normal operation, start-up, planned and emergency
shut-downs and, where appropriate, for maintenance and decontamination of plant.

(f)

Emergency procedures are available.

TEAM
The Commissioning Manager is responsible for the completion of Hazard Study 4, with or without the
assistance of site or project resources as appropriate.

4.3

TIMING
Hazard Study 4 should be completed prior to the introduction of process materials. Under process
materials is also understood:

(a)

Nitrogen (e.g., for purging, blowing or leak testing).

(b)

Steam (e.g., for steam blowing).

(c)

Air (e.g., for blowing or leak testing).

(d)

Water (e.g., for flushing or leak testing).

(e)

Chemicals (e.g., for chemical cleaning, pacifying).

DOCUMENTATION
The Commissioning Manager should ensure that the Project Safety, Health and Environment Dossier
(STD/F/01022) has been updated and is available on the plant.
A list of any outstanding actions which need to be completed should be produced and added to the
Dossier.

METHOD
This Study will require many actions to be carried out by a number of people, all coordinated by the
Commissioning Manager, and will not normally involve many formal meetings. Use of the standard
form will help the Commissioning Manager ensure that a thorough check of the installation is achieved.
The following is suggested:
(1)

Use the form (STD/F/01021)


(The list may need adapting or extending in some circumstances.)

(2)

Check that the hardware is as the design intended and that the software requirements are
completed.

(3)

Check that what was called for in earlier hazard studies and engineering change orders has
been implemented.

(4)

Check that modifications made during construction/commissioning have been formally


reviewed and documented.
