Travelers' diarrhea is the most common illness in persons traveling

from resource-rich to resource-limited regions of the world


ravelers' diarrhea refers to diarrhea that develops in individuals from
resource-rich settings during or within 10 days of returning from
travel to resource-limited countries or regions. For epidemiological
purposes, it is frequently categorized into three forms: classic,
moderate, and mild. These forms of travelers' diarrhea are defined
as follows [3]:
Classic passage of three or more unformed stools in a 24 hour
period plus at least one of these other symptoms: nausea, vomiting,
abdominal pain or cramps, fever, blood in stools
Moderate passage of one or two unformed stools in 24 hours
plus at least one of the above symptoms or more than two unformed
stools in 24 hours without other symptoms
Mild passage of one or two unformed stools in 24 hours without
other symptoms
The classic "turista" due to enterotoxigenic Escherichia coli (ETEC)
generally produces malaise, anorexia, and abdominal cramps
followed by the sudden onset of watery diarrhea. Very frequent
stools are uncommon. Nausea and vomiting also may occur.
Typically there are no symptoms of colitis such as blood or pus in the
stool. Patients may develop a low grade fever.
Even when other bacterial agents such as Campylobacter jejuni and
Shigella spp are implicated, the symptoms initially experienced by
the traveler are generally similar to those seen with ETEC. However,
infections with these organisms may progress to include symptoms
of colitis, such as fever, tenesmus, urgency, cramping, and bloody
diarrhea.
The diagnosis of travelers' diarrhea is made in an individual who
develops unformed stools during or shortly after returning from
travel to a resource-limited setting. Travelers' diarrhea is typically
self-diagnosed.
Low-grade fever
an oral temperature that is above 98.6 F (37 C) but lower than
100.4 F (38 C) for 24 hours.

Sulfasalazine is used in the treatment of inflammatory bowel
disease, including ulcerative colitis and Crohn's disease. It is also
indicated for use in rheumatoid arthritis and used in other types of
inflammatory arthritis.
Sulfasalazine metabolizes to sulfapyridine. Serum levels should be
monitored every three months, and more frequently at the outset.
Serum levels above 50 g/l are associated with side effects. In rare
cases, Sulfasalazine can cause severe depression in young males. It
can also cause temporary infertility. Immune thrombocytopenia has
been reported.[9]
Sulfasalazine inhibits dihydropteroate synthase, and can cause
folate deficiency and megaloblastic anemia.[10][11][12] and various
other undesirable effects.[13]
Sulfasalazine can cause hemolytic anemia in people with G6PD
deficiency.[14]

Sulfasalazine may cause stomach upset, nausea, vomiting, loss of


appetite, headache, dizziness, or unusual tiredness.
In Crohn's disease and ulcerative colitis, it is thought to be an
antinflammatory drug that provides topical relief inside the
intestine. It does this via a number of mechanisms such as reducing
the synthesis of inflammatory mediators known as eicosanoids and
inflammatory cytokines. However, compared to glucocorticoids
(another class of drug used in the treatment in inflammatory bowel
disease), sulfasalazine is only a mild immunosuppressan
