2014 Vendor Agreement

This agreement is required for all vendors participating in The Witches Cotillion Charity Ball 2014
held on Friday October 31, 2014, at The Riverview Ballroom, 13 Port Street, Hudson, MA 01749.

By signing this agreement, I, the vendor and any staff, agree to abide by the following:

1.I understand that my spot will not be guaranteed until my non-refundable $50 payment is recieved.
Please make all checks payable to "Charmed/Tara Hinckley" and mail payments to: Kristina Faxon, 35
Hosmer Street, Unit 21, Marlborough MA, 01752. The deadline for payment and signed contract is
October 1, 2014.

2.I understand that I and my staff must arrive at the designated vendor set up time of 6:30 PM and be
ready to go when doors open at 7:30 PM.

3.I agree that I and my staff are required to dress for the occasion. I understand that this is a
Samhain/Halloween event and that appropriate costume or black attire is preferable.

4.I understand that The Witches Cotillion Charity Ball 2014 is for adults 21 years of age or older.

5.I understand that as a vendor I am required to bring my own standard 6 - 8 foot rectangle table. I may
ask Lady T to provide extra chairs if need be. If I do not have a table or cannot bring one due to
transportation or other constraints, I agree to contact Lady T immediately so she and her staff members
can help find one for me. I understand that this courtesy will be first come, first served, or for
whomever has the greatest need. If I and another vendor choose to share a table, I understand that I am
still required to fulfill any and all other obligations of this contract, including full payment of the $50
vendors fee.

6.I understand that as a vendor I am required to maintain a clean and orderly area. I understand that I
must bring a table cloth or covering for my table and a sign promoting my business for my table.

7.I understand that as a vendor, I and all members of my team are required to behave in a professional
and courteous manner at all times. If we have a problem with a customer or another vendor, I agree to
contact Lady T immediately and she will handle the situation for me. However as a vendor, I do retain
the right to refuse service to anyone and as such I do not have to tolerate, rude, ignorant, disrespectful
or foolish behavior. I agree to report any such disturbance to Lady T and allow her or her staff to handle
the situation.

8.I understand that as a vendor I am an independent contractor and am responsible for my own sales
tax, federal taxes, and all other taxes I or my company may be required to pay. I agree that neither
Charmed, The Witches Cotillion Charity Ball 2014, nor Lady T or her staff will be held liable or
responsible for any required filing that I may fail to do.

9.As I vendor, I understand that I am to retain all monies received for my goods and services and
that neither Lady T or her staff, nor The Witches Cotillion Charity Ball 2014, are entitled to any
percentage of said monies paid to me for my goods and services.

10. I understand upon payment of my $50 vendor fee that I will receive (1) Free Ticket to the cotillion
beginning at 9 PM. I understand that discounted rates are available and will call or email Lady T for
these rates.

11.I understand that I and my staff will refrain from drinking any type of alcoholic beverage during the
hours of 6:30 PM to 1 AM, during The Witches Cotillion Charity Ball 2014. There will be a cash bar
available but will be for paid guests of The Witches Cotillion Charity Ball 2014 only.

12.I understand that the use of any type of illegal drugs is strictly prohibited during any part of The
The Witches Cotillion Charity Ball 2014. I understand that if I or my staff is under the influence of any
illegal drug, I and my staff will be asked to leave immediately and as a result my vendor table will be
closed and my vendor registration fee and any additional monies paid for tickets will be forfieted. I
understand that I, and any apart of my team, will not be allowed to stay at The Witches Cotillion
Charity Ball 2014 as a guest. I also understand that I will be banned from participating in future events
held by Lady T and Charmed.

Additional Information:
If you would like to pay your vendor fee by credit card, please contact Kristina through email at
kristinafaxon@hotmail.com and we can arrange something for you.
Tables will be assigned according to the order in 'witch' we receive your required information -
contract and vendor fee payment.
We will do our best to stagger vendors so no two similar products or services are next to one another.
If you have a location preference or another vendor that you would like to be next to please indicate so
when completing this agreement.
Also, if you have specific requirements for your table, please indicate so when completing this
agreement. If you need electricity, please bring an extension cord and a surge protector.
We will be raffling off many items, baskets, and gift certificates. A raffle donation from your business
would be greatly appreciated. Please include your business card and pertinent information with the
product or service. A maximum dollar value for the donation should be no greater than $25.
