A community health plan is never the work of anyone alone.

Many different people, in their

different ways, have given their heart whelming full support in making this activity a magnificent
experience. The group would like to extend our appreciation especially to the following:

Above all, we thank God the Father for the wisdom that He has bestowed upon us and the
strength that kept us going.

Our deepest sense of gratitude to our pre-school coordinator, Dr.Bautista??, who offered his
continuous advice and unfailing interest throughout the course of this activity;

To Dr. Wilhelmina Mercado for his guidance and efforts as our head;

To Principal??? for her continual support and encouragement throughout the activity;

To Ms. O. and all school staff, who sincerely devoted their time and service in every activity and
task. The implementation of our projects would not have been possible without their truly
unselfish efforts;

To all the students, we wholeheartedly thank you for your kindness, willingness and patience, for
the time with laughter weve spent with you. Those four weeks has taught us a lot and will
always be remembered;

To our beloved parents who gave unconditional support, for inspiring us in pursuing our career
