Job Title: Registered Nurse / Dissemination Training Specialist for Family

Connects International

JOB DESCRIPTION

Occupational Summary:
Lead Home Visiting Nurse Trainer for National Dissemination

This position serves as the lead nurse trainer for the dissemination of the model (Family
Connects) to Family Connects sites throughout the United States. Serving as a lead trainer
for the nurse visit protocol and quality assurance process, this position will coordinate nurse
home visitor training in Durham, NC, travel to dissemination sites for on-site audits, and
provide regular consultation to sites. Throughout the implementation process, the position
will provide regular phone/internet-based consultation to dissemination sites and document
ongoing progress toward program certification.

The position is housed within the Center for Child & Family Health in Durham, NC and is a
key position within the Family Connects International office. Experience in, or openness to,
training in implementation science and protocol driven practice is critical to this position.
The position will also involve training as a Family Connects home visiting nurse if the
applicant is not already certified for the Durham Connects program. Travel will vary and may
require up to 5-10 days per month at various times throughout the year. On-site review for
replication programs is a priority for this position; the review involves quality assurance for
the local home visits on-site assessment of the overall core program components.

Secondarily, the position involves practice in the Family Connects home visiting program,
providing home visiting to families with newborns in Durham County, NC as a basis for
training and quality assurance rather than carrying an assigned caseload. The goal is to
support and assess family needs in 12 domains of physical and psychosocial well-being and
facilitate referrals and connections to community agencies to support families in these
areas. Ability to communicate effectively with others and nursing experience in
maternal/child health, home visiting, and lactation education is desired. Fluency in Spanish
desired but not required.

Work Performed:
Dissemination Specialist / Lead Nurse Trainer Role
Provide training and quality assurance in the Family Connects home visiting model
with expertise in implementation science;
Understand and use the cascading model of program dissemination for replication at
other national/international sites;
Participate with the Family Connects International team participating in all relevant
practice and policy meetings;
Apply web-based communication and training methodologies as used by the team;
Apply principles of community alignment and data and monitoring to dissemination
practice;
Participate in continuing education as needed for Family Connects core

competencies;
Provide on-site audits for program implementation in dissemination sites;
Document supervisory and training contacts with dissemination sites; and
Participate in implementation science training, as needed

Nurse Home Visiting Role


Provide a limited number of Durham Connects home visits in order to maintain and
enhance skills as well as contribute to the clinical need in Durham.
Keep records of client observations and activities as required. Document information
for family assessments and program evaluation. Record information via electronic
medical record.
Provide a health and physical assessment of mother and baby in the home to
determine well-being and answer family questions about newborn behavior and
needs.
Assess risk factors for child maltreatment, including depression, substance abuse,
and domestic violence. Exercise clinical skills in appraising situations and making
decisions in collaboration with the family.
Establish a trusting relationship with families during in-home visits, engaging with the
family to establish a family support plan with goals, objectives, and activities that
address family strengths and needs.
Work with team to ensure appropriate referrals and connections to community
resources as needed.
Participate in training in program core competencies critical for home visiting
expertise.
Apply working knowledge of parent-child interaction, child-maternal health, child
development, and child abuse/neglect to help families improve parenting skills,
increase parenting supports, reduce stress, and address needs of parents; assist in
creating a health plan for entire family.
Maintain open home visit slots when not traveling for Family Connects International.
Respond to client needs through flexible scheduling of home visits, including
occasional home visits during evening or weekend hours.
Documentation of home visits is expected to be completed and letters to health care
providers sent out within 48 working hours of the visit.
Work cooperatively as an active member of the Family Connects International and
Durham Connects teams by attending team meetings, clinical supervision, staff
meetings, and other meetings as required.

Required Qualifications At This Level


Education / Training / Skills:

1. Minimum of a B.S. degree in nursing with NC RN license, or B.A. degree in a relevant


field with NC RN license, and at least one year of relevant nursing experience.
Training in implementation science/teaching experience highly desirable.

2. Must have and maintain current or compact licensure in the state of North Carolina.

3. Must maintain current CPR certification.

4. Must have comfort with, and interest in, and ability to, travel to replication sites with
capacity to travel nationally several times/year for 2 to 5 days for each on-site visit.

5. Must have a valid North Carolina drivers license and an automobile for use in making
home visits and more local dissemination sites.

6. Ability to work in a team with clinical and non-clinical staff.

7. Ability to work with culturally diverse populations.

8. Ability to move or lift at least fifty pounds.

9. Ability to work with Family Connects International leadership and Family


Connects/Durham Connects leadership to implement practice improvement and
clinical development plans.

10. Ability to work on weekends and flexibility for late afternoon home visits, infrequently
but as required by scheduling families.

11. Computer proficiency and comfortable using laptops as part of in-home assessment
and intervention and to assist dissemination sites to enter data and create reports
needed to document program implementation.

12. Preferred skills include (a) knowledge of implementation science, (b) clinical
experience related to domestic violence, substance abuse and/or depression, (c) one
or more years experience in child/maternal health, public health, home visiting, or
other related field, (d) Certified Lactation Educator or Consultant and (e) Spanish
language proficiency.
