‘The said esablishment is engaged in the refilling of used butane canisters with liquefied
petroleum gas IUS opensting ina suture made of conerete lumber, wood, end GI sheets which,
bas an estimated floor area of more or Iss than 100 square meters and is situsted on a Jot with a
land ares of more or less than 400 square meters, The structure is also utilized as an office,
sorage and refilling ea
‘There are three above ground tanks insalled on the seid Jot with a capacity of 1,000
sas 1.500 gallons and 2,000 gallons respectively.
_Am assessment wes made wherein the following deficiencies were noted as follows:

2 Violation of Sec 10.3.433 paragraph A Sub- paragraph 2 of RA 9514 which states
thst “No person shall inseall or maintain any LPG container, or operate any tank
vehicle used for the transportation of LPG without 2 permit tom the Department of
Energy (DOE) and other concemed govemment agencies.”
