We, students from Polytechnic University of the Philippines, taking up Bachelor of Science in
Business Administration major in Human Resources Development Management, are writing
to request permission to conduct a research study at your company. We are currently
making a thesis about Job Satisfaction of Employees and we would like to make your
company as the focus of our study.

If allowed, may disseminate questionnaires/survey forms to one hundred fifty-five (155)


employees randomly chosen in the chosen divisions/departments in your company. This
survey will not in any way disrupt your office functions and we will abide all of the rules and
regulation in your office.

The results of this study will remain confidential and will be purely used for educational
purposes only.
