AFFIDAVIT OF SURVIVING HEIRS

WITH WAIVER

WE, MA. DIVINA D. CONIENDO, single, of legal age, Filipino, and a resident of
Poblacion, Sibulan, Negros Oriental; BERNARDITA D. CONIENDO, single, of legal age,
Filipino, and a resident of Poblacion, Sibulan, Negros Oriental; REGINO D. CONIENDO,
married, of legal age, Filipino, and a resident of Toledo City, Cebu; GIOVANNIE D.
CONIENDO, married, of legal age, Filipino, and a resident of Talisay City, Cebu; and ZITA
C. ABELLA, married, of legal age, Filipino, and a resident of Poblacion, Sibulan, Negros
Oriental, after being duly sworn to in accordance with law, depose and state THAT:

We are the surviving heirs of FELECIDAD D. CONIENDO, who who died on

September 22, 2016 at Sibulan, Negros Oriental;

At the time of her death, FELECIDAD D. CONIENDO, left a business, Stall No. 84,
located at the Sibulan Public Market, Sibulan, Negros Oriental;

WHEREAS, WE, MA. DIVINA D. CONIENDO, BERNARDITA D. CONIENDO,

REGINO D. CONIENDO, and GIOVANNIE D. CONIENDO, have agreed and covenanted to
CEDE, TRANSFER and CONVEY unto our co-heir, ZITA C. ABELLA, her heirs and assigns
the full ownership and possession of the above of the above described business;

NOW, THEREFORE, for and in consideration of the foregoing premises, we waive

and renounce, as by these presents, our shares, rights and participations over the above-
described properties;

We are executing this affidavit in good faith to attest to truth that there are
no other heir or heirs except for the above-named heirs of the said deceased and for
whatever legal purpose it may serve.
