Next week we are doing an home visit where we will be doing
an Ages & Stages Questionnaire (ASQ-3). My EI mentor has
asked me to create a sheet with images of animals and other
objects for one section of the ASQ.
I am going to create a sheet with images of animals and other
objects for the communication section of the ASQ-3.
Objectives:
1. The picture sheet will help the the child remember
different animals, so she can name at least three different
items from a common category.
2. The picture sheet can be saved to be used for future ASQ-
3 for children between 45 months 0 days through 50
months 30 days.
3. The picture sheet will help the person filling out the
questionnaire see if the child is able to name items from a
common category and score him/her.
Description:
1 I will look up images of animals and other miscellaneous
objects on clip art on Microsoft Word.
4. I will create a picture sheet of eight images and then show
my EI mentor.
5. Once approved, I will print off the picture sheet.
Reflection: What went well is that the pictures where nice and clear, that
