1. SCOPO
Lobiettivo di questa procedura di definire il
flusso minimo di informazioni e di dati
statistici HSE tra Uffici, Basi, Cantieri e
Progetti controllati da SICIM S.p.A..
Come dichiarato nella Politica della Sicurezza,
Salute e Ambiente lAlta Direzione
impegnata nel monitoraggio dei risultati
ottenuti dallorganizzazione in modo da
garantire il raggiungimento degli obiettivi e il
miglioramento continuo.
La Politica della Sicurezza, Salute e Ambiente
indica i due principali strumenti per garantire
la conformit e il miglioramento: verifiche
ispettive, visite del Management e lutilizzo di
Indicatori condivisi e affidabili. Il presente
documento specifica lultimo dei due
strumenti, elencando e definendo quegli
indicatori e fornendo istruzioni su come
calcolarli, usarli e condividerli.
Il presente documento richiede che ogni
attivit svolta negli ambienti SICIM oppure
legata allesecuzione dello scopo di lavoro di
SICIM, sia essa preparatoria, svolta da
lavoratori SICIM o da Subappaltatori venga
inclusa negli Indicatori HSE, al fine di
ottenere una significativa visione dinsieme
statistica delle attivit dellAzienda.
Quindi lo scopo di questa Procedura di
fornire le istruzioni per il processo di raccolta
e inoltro di:
Leading Indicators; indicatori dei processi
che mirano alla prevenzione degli incidenti,
Trailing Indicators; indicatori che
descrivono gli eventuali incidenti con
lobiettivo di evitarne la ripetizione,
Key Performance Indicators infortunistici,
calcolati sulla base dei Trailing Indicators.
La procedura mira a creare un approccio
comune allinterno dellorganizzazione della
SICIM, essa non deve comunque mai essere
in contrasto con le diverse legislazioni

TITLE
1. SCOPE AND PURPOSE
The scope of this document is to outline the
information exchange of a minimum set of HSE
statistical data between Offices, Base,
Operational Sites and Projects controlled by
SICIM S.p.A..
Following SICIM HSE Policy, Company Top
Management is fully committed to monitor the
performances of the organization in order to be
able to check the accomplishment of the
objectives and to move toward continuous
improvement.
HSE Policy lists two main tools for monitoring
compliance and improvement: audit and
Management visits and the use of reliable and
common Indicators. The present document
refers to the latter tool detailing those Indicators
and giving information how to calculate, use and
circulate them.
It is required that any activity within SICIM
premises, related to SICIM scope of works as
well as preparatory to them, carried out by
SICIM workers subcontractors collaborators is
reflected in the HSE Indicators reporting in order
to be able to have a sound statistic overview of
the Company activities.
Hence, the purpose of this Procedure is to
provide details of the process of data collecting
and reporting of:
Leading Indicators; indicators of processes
that aim to prevent incidents,
Trailing Indicators; indicators that describe
occurred incidents with the aim to avoid
recurrence,
Key Performance Indicators relating to
Injury Rates, calculated on the basis of the
trailing indicators.
This Procedure aims to create a common
approach within SICIM Organization, it shall not
however be in contrast with national and
international regulation as well as specific Project

Result of an event which has caused wounds

An incident where no injury, ill health, or
fatality occurs, but that it may be provoked.
4. RESPONSIBILITY
GENERAL MANAGER (GM)
He is responsible for ensuring that the
Company procedure is implemented in all
SICIM Projects / Premises.
He participates with the all Top Management
in the definition of the HSE performances
objectives, Key Performance Indicators, and
communicates them to the Company
Management.
He is responsible for reviewing Company HSE
performances (KPI), communicate them to
the Company Management and ensuring
resources to reach the defined targets.
HEADOFFICE (HQ) HSE DEPARTMENT
Is responsible to lead the Process of the
present document, gather Indicators from
Company Sites and compile the Company
Performance statistics.
Is responsible to support the communication
task of the General Manager, releasing
periodically Company Indicators.
Is responsible to implement the present
procedure to all Sites, sharing any
information and supporting HSE Project
Managers.
PROJECT MANAGER (PM)
He shall ensure, with adequate resources,
that HSE Indicators are recorded and
reported to HQ HSE Dpt. within the Project of
his responsibility.
Reviews the HSE Indicators of his Project
against the targets and set-up together with

5. DESCRIZIONE DELLE
ATTIVIT
Questa procedura enumera i requisiti minimi
della raccolta e circolazione dei dati HSE fra
Cantieri, Uffici e Progetti.
Si ribadisce che clausole legislative vigenti
sono prioritarie alla presente procedura. da
notarsi inoltre che ogni obbligo contrattuale
deve essere soddisfatto in parallelo al
presente documento.
Il Responsabile HSE di Progetto dovr
utilizzare il database Odin e il modulo
0906-SI-021 HSE Internal Monthly
report per riportare tutti i dati richiesti da
questa procedura. Qualsiasi deviazione da
quanto richiesto deve essere preventivamente
concordata con tutte le parti in causa.
Nel Report mensile gli indicatori della forza
lavoro, i Leading Indicators e i Trailing
Indicators devono essere riportati suddivisi
tra SICIM e Subappaltatori.
La comunicazione mensile degli Indicatori
avviene via e-mail inviando entro l8 del mese
successivo il modulo 0906-SI-021 in
formato excel agli indirizzi:

HSE Project Manager an action plan to meet
the targets.
HSE PROJECT MANAGER
He is in charge to ensure the correct
implementation of this procedure and that
all indicators are properly reported, and to
send them to his Project Manager and to
SICIM HQ HSE Dpt. as required.
He shall harmonize any Client / Statutory
Reporting requirement with the present
procedure in order to streamline data
gathering.
He co-ordinates the implementation of
actions arising from an action plan.
5. DESCRIPTION OF
ACTIVITIES
This procedure shall be followed as a
minimum requirement for HSE data reporting
from Sites and Offices, Base, Operational
Sites and Projects. It is underlined that
provisions of current International / National
Legislation shall take priority over the present
Instructions. During operations, each
contractual obligation, furthermore, shall be
carried out in parallel with the above
Procedures.
HSE Project Manager will be able to use
Odin Database and form 0906-SI-021 HSE
Internal Monthly report to communicate all
data to be reported as per the present
procedure. Any deviation shall be discussed
and agreed on with the involved parties.
In the monthly report SICIM direct workers
Indicators, as well as Leading and Trailing
Indicators shall be reported split by the ones
coming by sub-contractors. All kind of sub-
contract shall be taken into account.
Every month the HSE Project Manager shall
collect Indicators and send them via e-mail,
using module 0906-SI-021, to the following
mails address:

sempre in copia nellinvio di queste
informazioni.
5.1. INDICATORI DI PRODUZIONE
I principali indicatori di produzione utilizzati
come denominatore nel calcolo dei KPI, vedi
5.5, sono: Media dei Lavoratori presenti,
Ore Uomo Lavorate, Km Percorsi (veicoli
trasporto persone e cose).
Questi Indicatori devono includere tutte le
attivit svolte o legate allazienda da
lavoratori SICIM e da tutti i subappaltatori
durante il mese corrente, incluso la fornitura
di servizi di manodopera.
Qualora informazioni complete non siano
disponibili, per esempio nel caso di mancato
rapporto delle ore uomo lavorate dai
subappaltatori, le ore uomo possono essere
calcolate come media dei lavoratori presenti
durante il mese moltiplicato per la media
delle ore mese lavorate da un lavoratore.
5.2. INDICATORI PROATTIVI
(LEADING INDICATORS)
Analisi specifica dei rischi
Il numero delle Analisi dei Rischi specifiche
per una lavorazione effettuate durante il
mese. Include tutti i tipi di documenti che,
data una nuova lavorazione o una lavorazione
di cui cambino le condizioni al contorno,
riportino, in qualsiasi forma, una sua
valutazione dei rischi.
Riunioni della Sicurezza
Il numero di tutte le riunioni, a qualsiasi
livello, che hanno avuto come argomento o
fra gli argomenti la Salute, la Sicurezza o la
Tutela ambientale sul luogo di lavoro. Deve
esistere ed essere mantenuto in cantiere
lordine del giorno o il verbale di riunione.

the Headquarter the report.
5.1. PRODUCTION INDICATORS
Average Manpower, Worked Man-hours
and Km Driven are the basic Production
Indicators used to calculate Key Performance
Indicators as explained in paragraph 5.5.
These Indicators shall include all work
produced in, related to the site, by SICIM
workers as well as by all sub-contractors
during the month, including all services
providing sub-contractors.
When no through data is available, i.e. some
sub-contractors, man-hours can be accounted
as per the average monthly manpower on site
time the average monthly worker hours.
5.2. LEADING INDICATORS
Job Hazard Analysis (JHA)
The number of task specific Job Hazard
Analysis, also called Job Safety Analysis done
during the month.
It should include also any kind of specific
Task Risk Assessment done, as long as it has
been reported and recorded in any kind of
written form.
Safety Meetings
Any kind of Meetings having as Agenda HSE
Issues.
Meetings shall be reported also if HSE is only
a part of Agenda, as long as the agenda is
recorded or the minutes of meeting are
available.

Riunione pre-attivit
Tutti gli incontri effettuati con una squadra di
lavoratori prima dellinizio del turno di lavoro
o prima di una specifica attivit lavorativa.
Solitamente incontri informali, non
verbalizzati di ca. 15 minuti che vedono
coinvolti i lavoratori, i capisquadra, i
responsabili della sicurezza e tutto il
personale di cantiere.
Ore di Formazione della Sicurezza
Il numero di ore complessive di formazione
fornita ai lavoratori, a qualsiasi livello, incluso
la Formazione obbligatoria.
Vedi anche paragrafo 5.1.4.
Verifiche Ispettive
Numero di Verifiche Ispettive ufficiali
effettuate dal Cliente, da Parti Terze.
Verifiche Ispettive Interne effettuate sulla
scorta di una specifica procedura di progetto.
Di ogni Verifica Ispettiva deve essere agli atti
il report (non deve essere comunque inviato
insieme agli indicatori).
Ispezioni della Sicurezza
Il numero di tutte le Ispezioni interne, anche
effettuate tramite Checklist utilizzate
periodicamente.
Il numero di tutte le Visite di Cantiere se la
Sicurezza, Salute e Ambiente stato uno
degli argomenti che la visita ha riguardato.
Denuncia di Atto o Condizione pericolosa
Il numero di ciascun atto o condizione
pericolosa denunciata o riportata in qualsiasi
modo, anche verbalmente, da un lavoratore.
Simulazione demergenza
Numero delle simulazioni di situazioni di
emergenza effettuate e formalizzate durante
il mese.
5.3. INDICATORI REATTIVI
Gli Indicatori Reattivi riguardano il numero di
Incidenti (Incidente Mancato e Infortuni)
accaduti durante il mese in oggetto.
Questa parte del Report mensile suddivisa
in due sezioni, ciascuna sezione riporta i

Pre-Job and Toolbox Meetings
Any kind of Toolbox Talks or Pre-Job
Meetings. If Toolbox Talks are done in
different groups each single group shall
account for 1 Toolbox Talks; i.e. if the
Toolbox talks are done in 4 groups every day
I will have 4 reported every day, 24 toolbox
talks every 6 working days week.
Safety Training Hours
Any kind of training given to workers at any
level, including Induction.
See paragraph 5.1.4 for more specific
information.
Audits
Number of Official Inspections and Audits
done by Client, any Third Party.
Internal Audit done as per specific Project
Procedure.
Each audit must result in an audit report, not
to be submitted together with the Indicators.
Safety Inspections
Any kind of Internal Inspection, resulting
from checklist routine use or walkabout done
on site.
Unsafe Act / Condition reports
Any kind of unsafe act or condition reported,
also verbally, as long as the resulting action
is recorded or followed in any way.
Emergency Drill
Any emergency drill performed during the
month.
5.3. TRAILING INDICATORS
Trailing Indicators report the number of
incidents (Near Miss and Accidents) happened
during the month, this part of the report is
divided into two sections, both sections report
the same incidents but accordingly to different

medesimi incidenti secondo due differenti
classificazioni. Ci implica che il numero
totale degli Incidenti riportati nella
Classificazione secondo le Conseguenze
sar sempre uguale al numero totale riportato
nella Classificazione secondo la Tipologia.
La comunicazione di questi Indicatori avviene
automaticamente se il cantiere ha adottato
Odin, deve avvenire mensilmente, con le
modalit illustrate al paragrafo 5, se si
utilizza unicamente il modulo 0906-SI-021
HSE Internal Monthly report.
5.3.1. CLASSIFICAZIONE SECONDO LE
CONSEGUENZE
Casi di Primo Soccorso
Tutti gli Infortuni minori che non richiedono o
potrebbero non richiedere lintervento di un
medico, ad esempio graffi, piccoli tagli,
piccole scottature, schegge.
Tali trattamenti sono considerati FAC anche
se sono forniti da un medico.
Casi di Trattamento Medico
Tutti gli Infortuni non classificati come LTI
(vedi sotto), o come RWC (vedi sotto) ma che
hanno comunque richiesto lintervento di un
medico.
Casi di temporanea restrizione delle
capacit lavorativa
Tutti gli Infortuni sul lavoro che hanno come
conseguenza una temporanea restrizione delle
capacit lavorativa. Include tutti i casi di
Infortuni che risultano in unassenza dal posto
di lavoro ma che sarebbero potuti risultare in
un temporaneo assegnamento ad altre
mansioni.
Il nuovo assegnamento deve essere una parte
significativa o porzione indispensabile del lavoro
regolare dellinfortunato. Come esempio si
prendano i casi Over-three-day injuries,
RIDDOR (legislazione inglese).

This means that monthly the total number of
incidents reported in the Classification by
Consequence shall be same as the total
number reported in the Classification by
Incident type section.
The notification of these indicators is done
automatically if the site has adopted Odin for
reporting, if not, it must be done on a monthly
basis, in the manner described in paragraph 5,
using the form 0906-SI-021 Internal
Monthly HSE report.
5.3.1. CLASSIFICATION BY
CONSEQUENCE
First Aid Case (FAC)
Term to define any minor injuries that usually
do not require medical care by a physician
(i.e. scratches, cuts, burns, splinters, not
embedded foreign bodies in the eyes, etc.)
and its eventual subsequent visits. Such
treatment is considered FAC even if provided
by a physician.
Medical Treatment Case (MTC)
A work related injury, which results in neither
lost time nor restricted work but which
requires treatment by a medical doctor.
Restricted Work Case (RWC)
A work related injury, which results in a
Restricted Work assignment on any day after
the day on which the accident occurred OR in a
absence from workplace but could have resulted
in a Restricted Work assignment.
The Restricted Work assignment must be
meaningful or a substantial part of a regular job
Example of RWC are RIDDOR Reportable over-
three-day injuries is one which is not "major"
but results in the injured person being away
from work OR unable to do their full range of
their normal duties for more than three days.

Tutti i casi di Infortuni sul lavoro che
conseguono nella temporanea inabilit
dellinfortunato di compiere un qualsiasi
lavoro regolare o una parte significativa o
porzione di esso (vedi RWC). I giorni persi
includono, le festivit ed eventuali giorni di
ferie, non includono il giorno dellinfortunio.
ref. I casi di Specified injuries to workers,
RIDDOR 2013 (legislazione inglese), sono
LTI:
Qualsiasi frattura con lesclusione di frattura
di dita;
Amputazioni;
Infortuni che potrebbero causare la
permanente perdita o riduzione della capacit
visiva;
Infortunio alla testa o al torso che causi danni
ad organi interni;
Ustioni o bruciature che:
Coprano pi del 10% del corpo;
Causino danni significativi to occhi,
sistema respiratorio o altri organi vitali;
Qualsiasi infortunio alla testa che richieda
trattamento ospedaliero;

Loss Time Injury (LTI)
Any work-related injury which renders the
injured person temporarily unable to perform
any regular Job or Restricted Work as per
RWC definition. In this case Lost Time
includes rest day, weekend day, holiday. The
day of the Accident is not counted when
calculating Lost Workdays; i.e. RIDDOR 2013
Specified injuries to workers:
fractures, other than to fingers, thumbs and
toes
amputations
any injury likely to lead to permanent loss of
sight or reduction in sight
any crush injury to the head or torso causing
damage to the brain or internal organs
serious burns (including scalding) which:
covers more than 10% of the body
causes significant damage to the eyes,
respiratory system or other vital organs
any scalping requiring hospital treatment
any loss of consciousness caused by head
injury or asphyxia
any other injury arising from working in an

Disability

Incidente con impatto ambientale Minore
1) Impatto sulle Acque:
Effetto limitato e localizzato sulla qualit
dell'acqua che ha un impatto minimo sulla
qualit o l'uso di tale acqua.
2) Impatto sul Suolo (Sversamenti / Rifiuti):
- Contaminazione trascurabile
temporanea di terreni senza alcun effetto
globale sull'uso o la qualit di quella terra.
- Danni molto limitati per la fauna e la flora
terrestre, riparabili naturalmente in un
breve lasso di tempo e senza danni alle
popolazioni di specie protette.
- Terreno agricolo o sito commerciale
colpito tra i 3 - 6 mesi, ma non dichiarati

Total monthly number of days lost in
consequence of an LTI incident other than a
fatal injury or permanent disability.
Near Miss (NM)
It is a hazardous Event/Incident which neither
harm to personnel nor damage to assets or
environment but had the potential to cause
such harm or damage.
Asset Damage Incident
Any work related incident which results in an
asset damage only, with no consequence to
personnel or to the environment. If the Asset
Damage is lower than the minimum
eventually report the Incident as a Near Miss.
Sometimes it is recommended to report
incidents, classifiable as Asset Damages, as
Near Miss; in this case we want to emphasize
more the characteristic of Near Miss -
escaped danger from a possible serious injury
- than the limited material damage suffered.
Minor Environmental Impact Incident
1) Impact on Water:
Limited and localised effect on water quality
which has a minimal impact on the quality or
use of that water.
2) Impact on the Ground (Spills / Waste):
o - Insignificant or temporary contamination
of land having no overall effect on the use
or quality of that land.
- Very limited damage to terrestrial fauna
and flora, which is naturally reparable
over a short timescale and no damage to
populations of protected species.
- Agricultural land or commercial site
affected between 3 - 6 months, but not

Le malattie professionali sono dovute
all'azione nociva, lenta e protratta nel tempo,
di un lavoro o di sostanze o di fattori negativi
presenti nell'ambiente in cui si svolge l'attivit
lavorativa (come la malattia del polmone nero
contratta dai minatori o il tumore tra gli
installatori di amianto).
Malattie e infortuni possono essere il risultato
di una stessa condizione (es. mal di schiena),
tuttavia la natura dell'evento scatenante o
l'esposizione differente. Un infortunio deriva
da un evento violento e istantaneo o una
singola esposizione, mentre una malattia

Road Traffic Incident (RTA)
Commuting incident
All RTA incidents which occur whereas a
worker is travelling between a place of
residence and the workplace and vice-versa.
Commuting incidents are not classified by
consequence and therefore are not counted in
the Trailing indicator calculation. They are not
included in TRCFR or RTAFR. Incidents in
which a worker is injured whilst travelling on
duty are not deemed to be Commuting
incidents, but as RTA, and are included within
LTI, RWC, MTC or FAC depending on the
consequence of the incidents.
Fire
Any accident resulting directly in the
occurrence of fire.
Security
Any Incident involving intentional and violent
use of force by any of the person involved
resulting in actual Injury and/or Damage to
Assets or to the Environment.
Occupational illness
Any work related event or activities which
resulted in a ill health, regardless of the time
intervening between the cause and the effect.
Occupational diseases are due to harmful, slow
and protracted action of employment,
materials or negative factors present in the
workplaces (such as black lung disease
contracted by miners or cancer among
asbestos installers).
Illnesses and injuries can include the same
conditions (e.g. back pain), however the
nature of the causative event or exposure is
different. An injury results from a single
instantaneous - violent event or exposure,
while an illness results from repeated
exposures/events or exposure over a period of
time (typical example: hearing loss).

gestione. Gli incidenti che si verificano
durante tali attivit devono essere riportati e
inclusi nelle statistiche di rendimento.
Le seguenti attivit sono considerate come
correlate al lavoro in quanto suscettibili ad
incidenti con impatto significativo, per i quali
dovrebbero essere in atto controlli da parte di
SICIM:
Tutti i lavori svolti da personale aziendale,
Tutti i lavori svolti da personale di
Subappaltatori nei cantieri SICIM, e
Tutti i lavori svolti da Subappaltatori in siti
che non sono di propriet di SICIM, per i
quali stato stabilito che siano necessari
controlli da parte di Sicim e del
subappaltatore, sulla base di valutazione
del rischio.
Per il personale Sicim, il lavoro comprende:
gli straordinari, la partecipazione a corsi,
conferenze e eventi organizzati dalla societ,
viaggi d'affari, visite sul campo, o qualsiasi
altra attivit in cui la presenza del dipendente
sia prevista dal datore di lavoro.
Qualora sia impossibile o inopportuno cercare
di imporre il proprio controllo di gestione su
un appaltatore, Sicim pu prevedere
eccezioni. Possono essere considerati come
esempio le aree dove i servizi dellappaltatore
non sono dedicati alla societ:

worker to hygienical or environmental

The latter applies if the hygienical or

consequence of the work site location (i.e.
where malaria is endemic) or under direct
control of Sicim (i.e. catering services)
Work Related
Those activities for which management
controls are in place, or should have been in
place. Incidents occurring during such
activities must be reported and will be
included in performance statistics.
The following activities are considered to be
work related, as they are susceptible to
incidents with significant impact, for which
Sicim controls should be in place:
All work by company personnel,
All work by Subcontractor personnel on
Sicim site-project, and
All work by Subcontractor personnel on
non-Sicim premises for which it is
evaluated on the basis of risk assessment
that Sicim and Subcontractor
management controls are required.
For Sicim personnel, work includes overtime,
attending courses, conferences and
Company organised events, business travel,
field visits, or any other activity where the
employees presence is expected by the
employer.
Where it is impossible or inappropriate for the
Company to seek to impose management
control on a contractor, exceptions may be
justifiable. Examples may be found in areas
where contractor services are not dedicated
to the company e.g.
Manufacturing of components in a factory
together with the manufacture of
components for other customers;
Construction at a contractors fabrication
site shared by other customers;
Delivery of goods or products to Sicim
locations by a contractor who is also
employed to deliver goods or products to
