III. Presentation of Problem/s

 - From the point of view of the Social Worker.

1. The minor is in need of a guardian over her person.

2. Who is the most qualified to be the guardian over the person of the
minor.

IV. Background Information

- Minor Arjoe Collado is a Grade 5 pupil at Northwestern University,

Elementary.
- The minor is healthy and a jolly kid.
- The father of the minor is a Canadian citizen who has another
family in Canada.
- The minor is living with her mother in their house at No. 151 I.
Thomas Street, Brgy. 1, Laoag City.
- That the minors mother was diagnosed with ENDOMETRIAL
CARCINOMA STAGE III-C CANCER and needs a constant
confinement in the hospital for medication and treatment.
- The minors mother is not able to do her obligation as mother to
the minor with respect to going to school and other activities due to
her sickness and staying in the hospital.
- With the absence of the minors mother, Frank Collado is
responsible enough in sending the minor to the school, attend all
meetings and accompanying the minor when the mother is in the
hospital.

V. Assessment

Considering that the mother is always at the hospital for medication

and treatment, the minor is in need of a person to look after her in every
activity she has to do. And the Frank Collado is the proper person to do
such obligation. As we observe, the minor is comfortable when shes with
her uncle Frank Collado. We cant see any person to do such obligation as
the other relatives of the minor are far from their house and that they have
their own obligation to do.

VI. Recommendation

The Department of Social Welfare and Development therefore

recommend that Frank Collado be appointed as the Guardian over the
person of minor Arjoe Collado.
