Abstract
OBJECTIVE: Thepurposeofthiscrosssectionalsurveywastoevaluateattitudesandopinionsof
doctorsofchiropractic(DCs)specializinginpediatriccaretowardpatientsafety.
METHODS: TheMedicalOfficeSurveyonPatientSafetyCultureoftheAgencyforHealthcare
ResearchandQualitywasadaptedforproviderswhousespinalmanipulationtherapyandsent
outto2USchiropracticorganizations'pediatriccouncilmembers(n=400)betweenFebruary
andApril2014.Thesurveymeasured12patientsafetydimensionsandincludedquestionson
patientsafetyitemsandqualityissues,informationexchange,andoverallclinicratings.Data
analysesincludedapercentcompositeaverageandanonrespondentanalysis.
RESULTS: Theresponseratewas29.5%(n=118).Almostonethirdofrespondents'patients
werepediatric(17yearsofage).DCswithapediatriccertificationwere3timesmorelikelyto
respond(P<.001),butlittlequalitativedifferenceswerefoundinresponses.Thepatientsafety
dimensionswiththehighestpositivecompositepercentageswereOrganizationalLearning(both
administrationandclinical)andTeamwork(>90%).PatientCareTracking/FollowupandWork
PressureandPacewerepatientsafetydimensionsthathadthelowestpositivecompositescores
(<85%).Theresponsesalsoindicatedthattherewasconcernregardinginformationexchange
withinsurance/thirdpartypayors.Twoqualityissuesidentifiedforimprovementwere(1)updating
apatient'smedicationlistand(2)followinguponcriticallyabnormalresultsfromalaboratoryor
imagingtestwithin1day.Theaverageoverallpatientsafetyratingscoreindicatedthat83%of
respondentsratedthemselvesas"verygood"or"excellent."
CONCLUSIONS: Comparedwith2014AgencyforHealthcareResearchandQualityphysician
referentdatafrommedicaloffices,pediatricDCsappeartohavemorepositivepatientsafety
attitudesandopinions.Futurepatientsafetystudiesneedtoprospectivelyevaluatesafety
performancewithdirectfeedbackfrompatientsandcompareresultswiththeseselfassessed
safetyattitudes,aswellasmakefurtheruseofthissurveytodevelopacomparabledatabasefor
spinalmanipulationproviders.
