Tryouts open to all girls in grades 1-8 as of September 2017 Deadline for registration for tryout is Friday June 9 @5:00pm
Tuesday June 13 @5:15-7:15pm Preliminary tryout (first cuts will be made after this session)
Wednesday June 14 @5:15-6:45pm Final tryout (for those invited back after preliminary session)
These sessions are for those who are trying out for the Competitive teams with the Rockville Centre Recreation Department.
Tryouts for the training team program (Rising Stars and Gymnastics Club) will take place on June 19 and 20.
All gymnasts must wear leotard, no jewelry.

TRYOUT FEE: $50 cash, check, MC/VISA
Checks must be made payable to Inc. Village of Rockville Centre
Tryout registration deadline is June 9 at 5:00pm
Note: If child does is not chosen for United or Ultimate team, there will be no further
tryout fee for Rising Stars/Club tryout.

Two afternoon/evenings per week - 2 (gr. 1-3) or 3 hours (gr. 4-8) each practice Tuesdays and
Wednesdays plus one Friday 5:45-7:45 per mo. for all grades

Tryout Registration: Full tryout fee must accompany completed form at time of registration. Registration can be submitted to
the Rec. Center office during regular business hours 9:00am-5:00pm Monday through Friday and 9:00am-12Noon on Saturdays.
Deadline is Friday June 9. If you have any questions, coaches will be available to speak with you by appointment- email Heather
at HKnoch@rvcny.us. Registration will not be taken after the registration deadline of June 9!

I have read the information above and if chosen for either the Ultimate or United team, my child is prepared to make this commitment from September 2017
through June 2018. I understand that monthly payments are due at the first practice of each month. I am aware that my child is expected to attend all
practices and meets.

I assert that my child is healthy and able to participate in the sport of gymnastics. I am/am not attaching a list of limitations
or restrictions.

The Village of Rockville Centre Recreation Dept. recognizes its obligation to make our students and their parents aware of
the risks and hazards associated with the sport of gymnastics. Students may suffer injuries, possibly minor, serious or
catastrophic in nature. By its very nature, gymnastics carries a risk of physical injury and this risk cannot be eliminated.
Parents should encourage their children to follow all the safety rules and the coaches instructions. I give permission for the
RVC Recreation Dept., its coaches and other staff members to take whatever emergency measures are judged necessary
for the care and protection of my child while under its supervision. In case of a medical emergency, I understand that my
child may be treated by the local emergency resources (police, rescue squad) if deemed necessary. I waive and release all
rights and claims for damages that my child or I may have against the Village of Rockville Centre or its representatives. With
the above in mind, and being fully aware of the risks and possibility of injury involved, I consent to have my child participate
in the program offered by the Recreation Dept. I am signing this waiver of liability, consenting and releasing the Village from
all liability claims and causes of action arising out of activities at the Recreation Dept.
