I am herewith informing that the Data Entry Operator post was redeployed from CHNC,
Bhimavaram to Community Health Centre, Bhimavaram for the purpose of E- aushadi at Community
Health Centre, Bhimavaram, W.G.District. Further, it is informing that the Data Entry Operator, who
was appointed on contract basis in the above post, is on UN authorized absence from the month
ofaugust-2016 and the same was also intimated to the District Medical and Health Officer,
W.G.District.

In this connection, I am herewith recommending E.Srinivasa Rao working as Data Entry

operator N.T.R Vaidya seva at Community Health Centre, Bhimavaram for the above post on contract
basis, as he is rendering his services to Community Health Centre, Bhimavaram since 15 years in
various post on out sourcing basis and also informing that the services of the above post are very
essential to Community Health Centre, Bhimavaram for E- Aushadhi and other data entry works, as
we are facing very difficult for these works without data entry operator.

Further, it is informing that Sri.Pulaparthi Anjaneyulu garu, Honble Member of

Legislative Assembly, Bhimavaram Constituency has recommended sri.E.Srinivasa Rao for the
above post, vide reference cited above (letter enclosed).

In view of above, I am herewith forwarding the bio-bata of Sri.E.Srinivasa Rao along

with the letter of Sri.Pulaparthi Anjaneyulu garu, Honble Member of Legislative Assembly,
Bhimavaram Constituency for favorable order in view of E- Aushadhi and other data entry works at
Community Health Centre, Bhimavaram, W.G.District.
