Double Branch Elementary School

Parent/Teacher Conference Form

Date: April 18, 2017

Students Name: Student A Grade: Kindergarten Teacher: Ms. Walter

Type of Conference: Face to face Conference Attended by: Ms. Walter, Ms. Dula & Mrs. A

Purpose of Conference: To discuss Student As progress so far for the 2016-2017 school year.

Recommendations:
In reading, Student A is currently a 1G. He is struggling to read 1G books fluently & with accuracy. Student A
does not always pay attention to the words he is reading, but instead, uses just the pictures and what he thinks it
might say. Student A should continue to practice nightly on the Bookshelf app reading 2-3 1G (green) books to
help build his fluency and allow him to practice. After reading, Student A should practice retelling a storybook
or giving the main topic & key details of an informational text. By the end of the year, the expectation for
Kindergarten is that students are reading at the beginning of the 2G level. If Student A practices nightly & really
focuses on his 1G books for the next few weeks, Ms. Walter feels like Student A will be where he needs to be.

In writing, Student A is currently on level. He is able to spell phonetically and get his point across. Student A
should continue to work on using nice, neat handwriting and using finger spaces. When writing, he needs to
make sure he is using correct punctuation at the end of each sentence. He also needs to make sure he is on topic
with his writing. Student A can sometimes write about what he wants to write about instead of the topic given
by the teacher.

In math, Student A is currently on level. He is able to show understanding of the basics of addition &
subtraction. He should continue to practice with addition & subtraction fluency.

Behaviorally, Student A does his best to follow the Rancher Rules, but can sometimes be very easily distracted.
He needs lots of reminders to stay focused on his work. Ms. Walter would like to see him staying focused &
following the directions of an assignment. Ms. Walter & Ms. Dula are also working with Student A on sitting in
his seat the proper way to see if this will maybe help him stay focused.

Results:
Ms. Walter & Ms. Dula are going to continue working with Student A on the 1G reading level. Student A will
continue to practice at home as well using the Bookshelf app. Another conference can be scheduled after
progress reports if needed.
