Important in the case of immature cataract
Rx of immature cataract depends upon age and occupation
- Place

COMPLAINTS:
Defective vision in both eye ( duration 1 month - senile cataract )
Sudden diminision of vision in one eye duration traumatic cataract
Diminished vision in one eye duration complicated / congenital

H/O PRESENTING ILLNESS:


H/O painless progressive loss of vision eye duration [ if both eye more on side
duration ]
H/O Dayblindness nuclear cataract ; night blindness cortical cataract ( retinitis
pigmentosa )
H/O diplopia or polyopia ( due to irregular refraction )
H/O colored halor of lens ( due to hydration of lens )
H/O black spots
H/O trauma to eye ( traumatic cataract )
H/O pain, redness, watering, irritation, photophobia ( preceeding defective vision
complicated cataract )
H/O hypertension ( cortical cataract )
H/O diabetes mellitus ( metabolic cataract, snow fake cataract )
H/O prolonged administration of steroids for bronchial asthma, RA, eczema ( toxic
cataract )

ON EXAMINATION:
Examination of both eye togeather
Head posture
Both eye symmetrical
No ptosis
No proptosis
No lagopthalmus
No nystagmus

3. Lacrimal sac No swelling
region No pus point
No sinus / fistula
Skin not stretched or shiny
No fluid regurgitation ( if
dacrocystitis present cataract
surgery is not done )

6. Cornea Arcus senilis


7. Anterior Normal depth
chamber No abnormal constituent
8. Iris Color and pattern are intact
No iridoconosis
9. Pupil Centrally placed, round, 3mm
4mm both direct and indirect light
reflex present
10. Lens Immature cataract: ( cortical )
Grey color
Iris shadow present
Mature cataract:
Pearly white
No iris shadow
Nuclear cataract:
Brown / black
No phakodonosis
No subluxation / dislocation
No calcaneous deposits
No area of degeneration
No rosette shaped opacity

Seen by slit lamp examination after dilating the pupil


- Complicated cataract:
Bread crump appearance
Polychromatic cluster
- Hyper mature Morgagni cataract:
Milky white, brown, sunset appearance
Intumescent fluid cleft

VISION:
Finger counting immature cataract
Hand movements mature cataract
Perception of light mature cataract

DIAGNOSIS:
1. It is a case of bilateral senile cortical immature / mature cataract
2. Senile immature / mature cataract in left / right eye
3. Senile immature / mature cataract right eye
Pseudophakia / aphakia left eye
4. Traumatic cataract right eye
Normal left eye
5. Congenital cataract right eye
Normal left eye.
