RESUMEN
Las lesiones de la mucosa bucal son multifactoriales. Pueden ser de origen primario
o secundario, segn sean propias de enfermedades bucales o como parte del
cuadro clnico de diversas enfermedades del sistema nervioso, cardiovascular,
endocrino, dermatolgico y digestivo. El diagnstico de estas entidades es
bsicamente clnico.1,2
En las enfermedades digestivas, las aftas bucales aparecen como aftas solitarias o
estomatitis aftosa recurrente.
Esta lesin puede formar parte del cuadro clnico de las enfermedades siguientes:
gastritis crnica atrfica, duodenitis parasitaria, enfermedades inflamatorias
crnicas del intestino y sndrome de malabsorcin intestinal.
Los pacientes que sufren gastritis crnica atrfica pueden presentar lesiones
bucales como estomatitis aftosas recurrentes, relacionadas con estados carenciales
de vitamina B 12 y/o de hierro.
Duodenitis parasitaria
En la mayora de las entidades que cursan con malabsorcin hay prdida o atrofia
de las vellosidades intestinales, sobrecrecimiento bacteriano y trastorno
inmunolgico de base, que propician la malabsorcin de nutrientes esenciales para
el mantenimiento del trofismo de la mucosa del aparato digestivo, es decir, desde la
boca hasta el recto.22,23

Otras de las lesiones reportadas en estos pacientes son las lceras bucales que se
pueden presentar en el 20 % de los casos. 43
Se aprecian placas de color rojo, lisas, brillantes, con un epitelio adelgazado, las
cuales no llegan a ulcerarse, sin papilas filiformes y en las que se destacan las
papilas fungiformes; limitadas por una queratosis circundante sobreelevada de color
blanco-amarillento. Estas placas son de forma romboidal o circular, variables con el
tiempo, y recuerdan mapas heterogneos.

SUMMARY
Many of the lesions of the mucosa in the oral cavity are related to anatomical and
functional disorders of the organs of the digestive system. Some of them are part of
the clinical picture of the entity and others appear as complications of them.
Generally, they have an immunological, nutritional, or deficiency origin. These
lesions are difficult to be treated and that's why it is important to know the clinical
characteristics making easy an adequate interpretation, and to follow a suitable
therapeutic conduct at the same time. An updated review of the semiological
features of the oral lesions corresponding to the clinical picture of diverse digestive
diseases is presented, so that the general physicians, clinicians, stomatologists and
gastroenterologists be able to diagnose and give a better guidance and treatment
to the patients.
