Behavioral Intervention Plan Draft

Targeted Behavior: On Task Behavior

Intervention: Self-Monitoring Checklist; If student completes all tasks for checklist each day

then the student can earn a prize at the end of the week.

Time Period: Starting January 23rd; initial start time to be determined.

Data Points: Data points will be collected through the writing time for the student. The data

collection will be based upon a self-monitoring checklist for the student. The student will

monitor her own behavior based upon the number of tasks that is completed during the allotted

time for the period. There will be a baseline of 3 data points collected before intervention is

begun.

Data Collection Form: (An example of the data collection chart can be seen on the last page).

The data collection chart will be comprised of the time periods in which a task could be

completed. The time intervals will be in 10 minute increments. The student will then have a task

to begin/complete during this time. Once the time period is over the student will be able to check

off if they were on task the entire time. The teacher will monitor as well by the amount of

prompts given before student was able to complete the task. If the student completes all the task
will three or less prompts then the student can earn a prize at the end of the week. If student does

not stay on task then they will have to move onto the next task to be completed.

Baseline: The baseline will be completed after 3 data points have been completed. Once, the

baseline data has been finished, then the intervention will be put in place. The data points will be

based upon the number of task completed by student during the allotted time and the number of

prompts given.

Intervention: If the student does progress during this time we will move to an intervention. The

teacher will keep track of the students on task behavior. This will be seen by how many prompts

is given before a task was was completed. If the student has three or less prompts given after

each day for the entire week then they can earn a prize. The teacher will be able to easily scaffold

the amount of prompts needed for the student to earn a prize as well.
